  • Observed employees not wearing effective hair restraints.
  • There is a towel by the coffee maker and under the soup hot holding. All surfaces must be easily cleanable.
  • Both doors have gaps on the bottoms and side. If you can see daylight that means pests can enter. There are also several areas throughout the facility that would be considered harborage areas.
  • In the kitchen the laminate flooring is coming up. In front of the 3-compartment sink the flooring is actually taped down, and there's tape going into the storage room.
  • There are stained ceiling tiles in the dining room and in the storage room. There are also missing ceiling tiles by the video gaming and going to bathrooms. In the storage room there is an unfinished wall and exposed studs.
  • In the storage room there was a beverage fruit holder stored next to chemicals. I moved it next to the plates. Also by the 3-compartment sink there are spray bottles and chemical containers by glasses. As I explained the last time I was here chemicals must be stored below and away from food and food contact surfaces. I pointed these areas out to the employee and explained how the chemicals should be arranged.
  • Per employee, there are still other employees that have not gotten their food handlers license.

  • There was chili and baked potatoes leftover from last night that had been cooled down. The employee threw away the baked potatoes when she opened. The chili was in the process of being heated up in a crock pot. I told her she couldn't use the chili as it was cooled down and being improperly heated up. Again you can not cool down unless you become a category 1. There is no documentation as to what the owner has decided about the categories.
  • In the reach in cooler there is a ziplock bag of hot dogs date labeled 11-5. They were voluntarily disposed of.
  • In the reach in cooler there was eggs stored over several ready to eat foods. They were moved to the bottom.
  • Observed employee improperly washing their hands. Explained that she must use the paper towel to touch the handles of the faucet.
  • Observed employees not wearing effective hair restraints.
  • There are numerous rags under the 3-compartment sink. Several are very dirty. The employee on duty thought they may be drying. You should not have an accumulation of rags like that. This is clearly a few days worth of rags.
  • the third basin of the 3-compartment sink leaks. There is a bucket underneath to catch dripping water.
  • The door at the top of the ramp has a gap at the bottom. There is weather stripping on the bottom of the door, but the weather stripping attached to the ground is missing in places. I pointed this out to the employee. It must be repaired so that daylight no longer shows, that means pests can enter.

Also by the 3-compartment sink there are spray bottles and chemical containers by glasses. As I explained the last time I was here chemicals must be stored below and away from food and food contact surfaces. I pointed these areas out to the employee and explained how the chemicals should be arranged.
  • There are several unused cardboard beer boxes and cardboard from food that is stacked up in the storage room. I pointed this out to the employee. Having clutter such as this makes it hard to clean and gives pests a place to hide. Also there is an unused upright freezer in the storage room. If you do not intend to use it, it must be removed.

  • In the reach in cooler in the kitchen there was cooked and cooled down spaghetti and meatballs. There was no time of onset of cool down. It was voluntarily disposed of. The facility is a category 2 meaning you can not cool down. There are also boiled eggs in the reach in cooler and used in tuna salad. If there are any leftovers they have to be disposed of or taken home that day.
  • The meatballs and sauce were voluntarily disposed of.
  • There is a cork board with tacks on the wall over coffee maker, cups, coffee filters. The cork board must be moved to an area away from food and food contact surfaces.
  • Observed employee come in from smoking and not wash his hands. Explained hygiene and when to wash hands. The employee had a cup he was drinking from. Explained he must drink from an enclosed container, preferably with a lid and a straw. Also the handsink closest to the storage room appears to be used as a dump sink now. The employee reaffirmed this that it is used as a dump sink and they only use the hand sink on the other side of the kitchen. Explained if a sink is intended as a hand sink it must only be used for hand washing.
  • There are rags scattered throughout the kitchen. You can only have rags stored in sanitizer.

  • The quat sanitizer in the spray bottle was >400ppm. Explained how to make it per the label on the bottle. Also suggested making the sanitizer in the third basin of the 3-compartment sink and then putting it in a bucket/spray bottle.
  • There are several unused cardboard beer boxes and cardboard from food that is stacked up in the storage room. I pointed this out to the employee.   • Found personal foods in the reach in cooler. Discussed labeling as personal and separating them from the facility's foods.
  • The soda guns and holders have buildup in them.
  • Found missing coving in the kitchen. I pointed this out to the employee.
  • There are places where the flooring is starting to come up.
  • Found stained/damaged ceiling tiles.
  • On the side of the building there was several garbage bags full of cans. Per employee they are usually picked up once a week, but this week weren't picked up. Discussed putting in garbage cans so they don't attract pests.
