  • Observed unlabeled cooled mushrooms stored in container in the Migali cooler in the kitchen. Staff voluntarily discarded mushrooms.
  • Observed rusty shelves in the Migali cooler in the kitchen. Also, the beverage coolers in the bar area have bars that are rusty. Repair. The inside of the kitchen cabinet is not sealed. Seal or paint.
  • Observed duct tape on outside of ice machine. Remove tape and repair machine.
  • Concentration of sanitizer (chlorine) sanitizer buckets exceeded 400 ppm. The concentration was corrected on site by staff. Final concentration was 200 ppm.
  • Observed build up of dirt on the storage shelf by the white stove in the kitchen.Also, the inside of the kitchen cabinet by the grill has build up of dirt (and is sticky to the touch), especially in pot storage area. The inside of the ice machine has dirt build up; also, the Migali cooler and freezer have dirt build up. Defrost and clean.
  • Observed build-up of dirt build-up of dirt on floors, especially in the bar area- floor cover was holding dirt in place, remove and clean floor. Also observed dirt build up in the following areas: under, behind and around equipment (grill, coolers, ice machine) and in many corners including the 3 compartment sink area and soda line. Observed debris on the floor in the cabinet. Clean floors.
  • Observed broken floor tiles in the bar area, some were totally missing. Replace/repair.
  • Observed holes in multiple places on walls in the facility. Repair. Car board wall of cabinet is peeling away. Repair. The ceiling in the kitchen (above the grill area) is cracked and peeling away. Repair.
  • Observed chemicals (cleaning chemicals) stored on ice machine. Chemicals were immediately removed by staff and stored separately.
  • Observed a lot of clutter and extraneous items in the back area of the kitchen, some of which may preclude proper cleaning. Maintain only necessary articles on premises. Remove clutter.

  • A couple of the soda lines in the back area were sitting on the floor. Tie them up at least 6 inches off the floor.
  • Paint or replace the rusty shelves in the Migoli stainless fridge.
  • Repair the holes in the bar front and top of the ice machine (where duct taped FRP is).
  • The spice shelf in the kitchen had a build up of food debris. Clean and sanitize it.
  • One of the pizza ovens had a build up of food debris. Clean and sanitize it.
  • The outlet on the East wall by the grill had a build up of grease. Clean it.
  • Repair or replace the rickety 3-compartment sink and loose faucet in the bar area.
  • Tile the floor area under the entry rug where the concrete has been repaired and replace the broken tile in front of this area. Replace the missing floor tiles in the bar area. Paint or seal the concrete floor in the walk in cooler. Repaint the chipped concrete floor in the back storage areas.
  • Found build-up of debris on floors. Clean the floors in storage and bar areas.
  • Replace the missing ceiling above the Migoli cooler and freezer and repair the holes in the ceiling and walls in the single use paper product/beer storage area. Repaint the peeling walls in the mop sink area. Repair the large hole in the wall, missing cinder block behind the Migoli freezer in the kitchen. Repair the gaps in the walls in the walk in cooler.
  • Cover the foil-backed insulation in the walk in cooler ceiling with an easily cleanable, non-absorbent surface.
  • The fan in the walk in cooler was dusty. Clean it.

  • Found opened sandwich ham and turkey in the white fridge, unlabeled. Also, found unlabeled individual sour cream, salsa, sauce in cooler in kitchen. It must be labeled with the date it was opened and used within 7 days.
  • Found sandwich turkey and ham the was unlabeled in the white fridge in the kitchen, sliced lemons and limes in the bar cooler labeled 2/13, hard boiled eggs in the stainless fridge in the kitchen labeled 1/16, and individual salsa and sour cream that were not date labeled. These foods were discarded. They must be date labeled and used within 7 days.
  • Found orange juice mix and opened cranberry juice in the roll top cooler behind the bar at 46F. Maintain potentially hazardous cold food and drinks at 41F or below. Do not use this unit to store potentially hazardous foods and drinks in it until it has been repaired and is maintaining cold foods and drinks at 41F or below. Contact TCHD for a re-inspection when repaired. Drinks were discarded by manager.
  • Did not find thermometers in many of the refrigeration units.
  • Found lemons and limes stored with raw hamburger and eggs on the bottom shelf of the stainless fridge in the kitchen. Store lemons and limes above these raw products. Corrected-- lemons and limes were moved to upper shelf.
  • Found food stored under the water line in the back storage room.
  • Observed dirty soda lines inside the ice bin with clean ice for drinks at the bar. These lines must be kept clean so they do not contaminate the ice. Also, found soda lines sitting directly on the floor in the bar area. Tie these lines up off the floor to prevent contamination. Found a dirty mop/mop bucket up against the bag-in-box soda in the back storage area. Move the mop and bucket away from the soda.
  • Employee touched customer's unwrapped straw with her bare hands.
  • Observed employees not wearing effective hair restraints.
  • The following food contact surfaces were in disrepair: The lid to the plastic container of bacon in the white fridge in the kitchen was broken; the fan in the Migoli fridge in the kitchen was missing the cover and the other fan in this unit had been duct taped; the bar coolers, stainless coolers in the kitchen, and walk in cooler had rusty shelves; garbage bags were being used to cover food equipment and line the dish shelf; there were holes with exposed insulation in the freezer door of the Fridgedaire fridge/freezer in the kitchen; the ice machine door was broken; there was bare wood inside the beer walk in cooler and the FRP was popping off the walls; There was bare wood on the shelving unit outside the walk in cooler; the walk in fan was very dirty. Repair these items, paint or seal bare wood, and remove garbage bags from shelf and food equipment.
  • The following non-food contact surfaces need repaired: The bottom cover of the True cooler at the bar is missing; The cooler door seals on the bar coolers and white cooler/freezer in the kitchen are damaged; The top of the Migoli freezer and fridge were damaged/missing pieces; The door seal to the walk in cooler was duct taped; the ice machine has duct tape stuck to the top of it. Repair or replace these items. Paint or seal bare wood through out facility so that it is easily cleanable.
  • Found wet towels stored inside of the white cooler/freezer in the kitchen and under this unit.
  • The fryers and plastic tray in the fryer area had a build up of debris. Clean and sanitize these items.
  • The ice machine interior had a large build up of black debris. Clean and sanitize it.
  • The bar cooler and fridges and freezers throughout facility interiors had a build up of debris. Clean and sanitize them.
  • The following non-food contact surfaces had a build up of debris: The bottom of the True cooler at the bar; several fans throughout the kitchen and storage areas; Outsides of the cabinets under the popcorn machine; dust and debris build up on and around plumbing, wiring, equipment, and tops of coolers and freezers throughout the establishments. Clean and sanitize these areas.
  • Observed dirty containers stored as clean on top of white fridge/freezer in the kitchen. Wash, rinse, and sanitize these dishes again.
  • Observed leaks at the Men's restroom hand sink faucet, bar 3 compartment sink faucet, under the kitchen hand sink, from the water filter at the ice machine, soda lines in back room. Repair or replace these items. Repair or replace the broke faucet and properly install the 3-compartment sink at the bar. Remove the glass scrubber from the side of the bar sink that is labeled for hand washing. The drain line for one bar cooler is routed through the kitchen wall and into a bucket and the condensate line from the walk in cooler is draining into a bucket inside the cooler. Properly plumb these drain lines.
  • The vanity in the men's restroom is very water damaged and has a build up of black debris. Replace it.
  • There was a large accumulation of garbage, bags of aluminum cans, tires, rusted fencing around the outside of the facility. Remove and properly dispose of this waste at a permitted landfill.
  • There was a gap to the outside around the framing of the northeast back door to the facility through which daylight could be seen. Seal this frame to the outside. Contact TCHD when done.
  • Repair damaged floors throughout facility, including under rug in lobby.
  • Found build-up of debris on floors. Clean the floors throughout the facility, including the walk in cooler, kitchen, bathrooms, behind, under, and around equipment.
  • Replace missing base coving in bathroom and kitchen.
  • The walls and ceiling throughout the kitchen, bathrooms, bars, and storage areas had holes, bare wood, and damage. Repair all of these areas.
  • The walls throughout the bathrooms, bar, kitchen, and storage areas had a build up of debris. Clean and sanitize them.
  • Observed unshielded light in the walk in cooler.
  • There was an abundance of miscellaneous unused items and equipment stored inside this facility. Remove all unnecessary items.
