- Observed peeling paint on white equipment legs in kitchen. Repaint. Observed handsink in kitchen with cracked seal. Re-seal so that it is sealed to wall.
- Observed debris accumulation on the following items/surfaces: The 3 door refrigerator in kitchen, Prep counter in kitchen, and the milk dispenser (especially the seals) in the dining room. Wash, rinse and sanitize these items/surfaces and ensure routine cleaning.
- Observed clean equipment and dishes stored on rack located directly below a leaking area of the kitchen ceiling. There was a bucket located on the top rack to collect water that was next to clean dishes. Clean equipment must be stored away from possible contamination. The rack was moved at the time of this inspection.
- Observed peeling and chipping floor in walk-in cooler. Reseal or repaint.
- Observed peeling and chipping paint on ceilings throughout kitchen due to water damage. Per the kitchen manager the roof is scheduled for repair in the 3rd quarter. Observed peeling and chipping paint on the wall in the storage room located near the serving kitchen. Repair.

- Observed a package of hot dogs that were opened and dated 4/24/15. Once opened, hot dogs must be used or discarded within 7 days. Staff voluntarily discarded approximately 10 hot dogs at time of inspection.
- Observed peeling paint on white equipment legs in kitchen. Repaint.
- Observed debris accumulation on the following items/surfaces: microwave in satellite kitchen, Black beverage carts, and underneath the sink in the dining room. Clean these items/surfaces.
- Observed dishes stacked together wet on storage racks. Ensure dishes are air dried before storing.
- Observed peeling and chipping floor in walk-in cooler. Reseal or repaint.
- Observed spills on the floor in the walk in cooler. Clean.
- Observed dust accumulation on vents and ceiling in kitchen and in dry storage area by the dining room. Clean.

- In the satellite kitchen in the dining room, the counter had the coating coming off on several places. Underneath is wood that's not easily cleanable. In the dry storage room behind the dining room kitchen, there were wooden boards on the wall that appeared not to be sealed and a few small holes in the wall below the wood.
- Observed cleaned metal containers that were stacked together wet on the rack near the grill. Air dry all food contact items after washing.
- Observed a hole in the wall above the dish washing machine. Seal holes in wall. Ceiling tiles in kitchen next to dining room stained/ not easily cleanable.
- Dust/ black build-up on lower walls behind 3-compartment sink and grill.
