  • 4-501.12 - The cutting board being used to cut lettuce is heavily scratched and scored and stained. The food contact surface is no longer easily cleaned and sanitized due to condition.
  • 4-602.13 - The nonfood contact surface of the shelving for clean equipment had accumulations of grime and debris. The walk-in door handles and the reach-in door handles were not clean with accumulations of food and debris.
