The establishment came into the Health Dept and paid permit fee. Sup Garcia told the CFM to wait for EHS Kim to visit the establishment to reopen the business. Upon visitation, observed the establishment operating without Health Department's approval. Please be advised that you can only re-open upon the Health Department approval. The CFM has a certificate of his food safety examination. Obtain a CFM card (picture) at ORS at 703-533-7600 WITHIN 30 days or upon next routine inspection. Failure to complay will result in suspension of your permit.
