This visit was made to conduct a routine food safety evaluation. The Person in Charge was very helpful. The following issue requires attention:
1. Observed evidence of pest activity along the walls in the kitchen upstairs. Please have the pest management company make more regular visits and conduct regular cleaning. A follow up visit will be conducted in approximately 1 month to follow up on this issue.
Note: Food temperatures taken are foods being stored for the restaurant and not the catering facility. No food was being prepared at time of visit for any catering events and no food on site was for catering events.

  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: Observed that the coatings on the windows upstairs in the kitchen around the stove and refrigerator units are not smooth and easily cleanable.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material. Please have the moldings around the windows resurfaced.
  • Controlling Pests / Methods to Use When Pests Are Found
    Observation: Observed evidence of rodent activity in the kitchen upstairs along the walls behind the upright refrigerators. .
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. If pests are found on the premises methods shall be used to control their presence. Please have the pest control company make more regular visit and conduct more regular cleaning of the upstairs area. A follow up visit will be conducted in approximately 4 weeks to verify.

Update the EHP to include the following 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, Nontyphoidal Salmonella, E. coli, or Shigella).
The refrigeration units are improperly plugged into extension cords. Upgrade the electrical outlets to accommodate the refrigeration units.

  • Food Protection from Contamination / Package, Cover, Wrap (corrected on site)
    Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: rice, pancake mix, sugar, and collard greens. The foods were dscarded or covered during the inspection.
    Correction: Foods shall remain covered at all times.
  • Nonfood-Contact Surface/Corrosion Resistant/Nonabsorbent
    Observation: The nonfood-contact surface of the following equipment is not corrosion resistant, nonabsorbent, and/or smooth: the wooden storage shelves. Seal or paint the storage shelves.
    Correction: Non-food contact surfaces of equipment that are exposed to splash, spillage, or other foods that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
  • Equipment / Food-Contact / Visibly Clean
    Observation: The following equipment/utensils were observed soiled to sight and touch: oven, bulk containers, and the reach-in refrigerator.
    Correction: Equipment food-contact surfaces and utensils shall be clean to the sight and touch. A surface that is soiled cannot be effectively sanitized.
  • Equip/Utensils/Linens/Single-Service/Storage, Location
    Observation: Clean equipment were found stored on floor.
    Correction: Cleaned equipment and utensils, laundered linens, and single-service items shall be stored in a clean, dry location where they are not exposed to splash, dust, or other contamination, and at least 6 inches off the floor on NSF approved shelving units or dunnage racks.
  • Equip/Utensils/Linens/Single-Service/Original Package
    Observation: Single service containers were observed stored unprotected at the reach-in refrigerator..
    Correction: Single-service items shall be stored in a clean, dry location that is not exposed to splash, dust, or other contamination and at least 6 inches off the floor on approved shelving units or dunnage racks. In addition, the single-service items shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
  • Controlling Pests by Routine Inspection of Premises
    Observation: The premises is not being routinely inspected for the presence of insects, rodents, and other pests. I observed numerous flies in the preparation area.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Routinely inspect the premises to control the presence of pests on the premises.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the floors in the preparation area is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

03/26/2015Follow-up
This visit was made to conduct a risk assessement for the catering pop-up. The following must be corrected within 10 days:
- Fax email FPM car to A. Guerreiro (business card provided)
- Manager states that they have only done 1 event so far for the pop-up. All catering equipment and methods remain the same.
- No food for the catering permit observed at time of inspection.

  • Certified Food Manager: Certificate Process
    Observation: The Person in Charge (PIC) has a valid certificate of successful completion of an accredited Certified Food Manager (CFM) exam, but does not possess the Northern Virginia CFM card (manager on duty has expired Serv. Safe card. New chefs have been sent to get FPM cards and manager will also update his card)
    Correction: Obtain a Northern Virginia CFM card from ORS Interactive, Inc. within 10 business days. A copy of the Northern Virginia CFM card can be faxed to the Environmental Health Division at 703.746.4919. Failure to provide a copy of the photo identification card may result in further enforcement action.
03/23/2015Risk Factor
This visit was made to conduct a pre-opening for the catering permit at Del Ray Pizzeria. Facility will be cooking bacon, eggs and biscuits at Del Ray Pizzeria and transporting it to another facility. Food will be transferred hot in chafing dishes which will be properly insulated in foil/plastic and placed in an insulated pizza carrier. Once transported, food will be reheated if needed on an induction plate. Event will be taking place from 8am-11 am. All plates and utensils given to consumers will be disposable. All equipment (tongs, chaffing dishes etc.) will be returned to Del Ray Pizzeria to be properly washed, rinsed and sanitized. Dish machine at facility and reach in refrigerators are working properly (100ppm cl, 41.2 F, 41.9F). Menu will be emailed to A. Guerreiro.
