This visit was made to conduct a routine food safety evaluation. The Person in Charge was very helpful. The following issue requires attention: 1. Observed evidence of pest activity along the walls in the kitchen upstairs. Please have the pest management company make more regular visits and conduct regular cleaning. A follow up visit will be conducted in approximately 1 month to follow up on this issue. Note: Food temperatures taken are foods being stored for the restaurant and not the catering facility. No food was being prepared at time of visit for any catering events and no food on site was for catering events. *Repeat Observations are Subject to Civil Penalty

Update the EHP to include the following 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, Nontyphoidal Salmonella, E. coli, or Shigella). The refrigeration units are improperly plugged into extension cords. Upgrade the electrical outlets to accommodate the refrigeration units.

This visit was made to conduct a risk assessement for the catering pop-up. The following must be corrected within 10 days: - Fax email FPM car to A. Guerreiro (business card provided) - Manager states that they have only done 1 event so far for the pop-up. All catering equipment and methods remain the same. - No food for the catering permit observed at time of inspection.

This visit was made to conduct a pre-opening for the catering permit at Del Ray Pizzeria. Facility will be cooking bacon, eggs and biscuits at Del Ray Pizzeria and transporting it to another facility. Food will be transferred hot in chafing dishes which will be properly insulated in foil/plastic and placed in an insulated pizza carrier. Once transported, food will be reheated if needed on an induction plate. Event will be taking place from 8am-11 am. All plates and utensils given to consumers will be disposable. All equipment (tongs, chaffing dishes etc.) will be returned to Del Ray Pizzeria to be properly washed, rinsed and sanitized. Dish machine at facility and reach in refrigerators are working properly (100ppm cl, 41.2 F, 41.9F). Menu will be emailed to A.
