This is a reinspection for the abatement of the roach problem. Per the invoice from PCO, the site was serviced on 8/24/09. No roach activity was noted at servicing. I saw no evidence of activity today.

This reinspection is to ensure that means are being taken to abate the roach problem that is present. Per an invoice from PCO, the site was serviced on 7/28/09 and no live activity was noted at that time. Continue to clean up at night before leaving the facility/continue to have the establishment serviced on a regular basis by your pesticide control officer.

This is a reinspection for the abatement of a roach problem around the chicken display case. The holes around the pipes at the drive thru window's have been sealed. The manager on duty was unable to produce an invoice for the follow up treatment of the facility. I will return to the establishment on/around 8/7/09. Please produce an invoice for treatment at this time.

This is a reinspection for abatement measures being taken for the roach problem, especially near the chicken display case. Per an invoice from PCO, the site was serviced on 7/22/09. Seal any holes (near drive thru wall plate/around the sink pipes, etc). Clean up on a daily basis (shelvings, tray tracks, etc). I will make a follow up inspection on 7/29/09.

This reinspection is to verify that means are being implemented to abate the existing roach problem (chicken display). The PCO did not come on 7/20/09 but will be on site on 7/21/09. I will revisit the site on 7/22/09. Please produce an invoice from the extermination company upon my return.

Inspection (during lunch rush) did not find similar situation. Dining room tables reasonably clean, spot cleaning underway, hot holding temps of foods were satisfactory, soda machine working normally.
