    Observation: A spray bottle of cleaner is stored hanging on a wire shelf above a container of food that is out for prep.
    Correction: Store chemicals below and away from food and clean equipment.

A complaint was received that stated that on Saturday evening a couple ordered a BBQ chicken pizza with pineapple and bacon. They took the pizza home, ate it and became ill. The complaint also stated that the facility was extremely dirty.
A complaint and risk factors inspections were conducted. PIC was not aware of any complaints. The other staff member working during the inspection was the manager on duty on Saturday night - he was not aware of any complaints.
The employee health policy is posted in multiple locations. Staff were able to locate and indicate that the owner reviews the policy and is strict about Employee Health policy. According to PIC one employee called out sick with fever in the past two weeks. The employee reportedly had indicated that they might not attend work prior to calling in.
Please contact EHS if any other complaints are received.
Facility has been in business for approximately 20 years -> some areas are worn and/or in need of repair. Discussed opportunities for detail cleaning/ cleaning schedule revision with owner.
