• 0450 - Critical Employees observed handling ready-to-eat (RTE) food with their bare hands.
  Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.
• 0550 - Repeat Plastic drinking cup used for dispensing ice.
  Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135F.
• 3380 - Critical Chlorine solution in the three compartment sink - 0 PPM and does not meet the requirements of 21 CFR 178.1010
  Utilize chlorine strength at 50 - 100 PPM that meet the requirements of 21 CFR 178.1010 when applying to food contact surfaces

• 0220 - Critical Personal drinks found throughout kitchen without lid and/or straw.
  All drinks to have lids and straws. Owner discarded drinks.
• 0260 - Critical Moldy corn husks found in plastic bags on low shelf.
  Apparently these got wet from air drying dishes racked above. Owner discarded.
• 0470 - Critical Raw chicken stored above mushrooms and spinach in low boy. Raw eggs stored above flan in 2 door.
  Store raw foods beneath cooked and RTE foods. PIC relocated raw foods.
• 0520 - Container of lemons found inside ice bin.
  Consider ice to be a food. Do not store anything inside. Lemons were removed.
• 0550 - Ice scoops found with handles in ice.
  Keep handles out of ice.
• 0610 - Uncovered salsa and white sauce and chips.
  Protect.
• 0820 1 - Critical Rice on stove at 125 F.
  Maintain hot foods at 135 F or higher at all times. PIC turned burner on to reheat. Reheat to 165 F.
• 0830 - Critical Flan found without datemark. Cooked foods in walk in found without dates.
  Mark with prep date to expiration date. You have a total of 7 days which includes prep date. PIC agreed to review this requirement with workers.
• 1640 - Corrected During Inspection The in-use sanitizer water in the warewashing sink was observed to be heavily soiled or contaminated.
  Drain the basin(s) and refill with fresh water and cleaning agents/sanitizers added as required for effective cleansing, rinsing and sanitization of equipment and utensils.
• 2000 - Ice receptacle found uncovered on top of ice machine.
  Protect from dust and dirt, provide a lid or turn upside down.
• 2310 - Corrected During Inspection Critical The handwashing facility located at thebar is blocked, preventing access by employees for easy handwashing. A blender was placed in handsink.
  Access to the handwashing facility identified above is to be available during all hours of operation. Remove the blender preventing its use.
• 3330 - Corrected During Inspection Critical Unlabeled chemical spray bottle.
  Identify contents.

• 0060 - Critical No one passed demonstration of knowledge test.
  Answer page was given to owner to review. I will return to retest.
• 3090 - Vent fan in ladies room does not appear to be running at proper speed.
  Check unit.
• 3170 - Top of coving behind ice machine has gaps.
  Seal edges.
• 3180 - Fan cover in restroom are dusty.
