Reviewed the daily temperature log requirements for the refrigeration units. (Blank copies of the refrigeration temperature logs were provided).

Observation: New employees do not possess a valid food handler's card.
Correction:
Employees who do not possess a valid food handler card are required to obtain one before the September 18, 2015 follow-up inspection is conducted.

Observation: The prepared ready-to-eat (RTE) foods in the refrigeration units are not properly dated for disposition.
Correction: Mark the name and ""consume by"" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
- Cooling, Heating, and Holding Capacities
Observation: The sandwich make unit is not maintaining a cold-holding temperature below 40 degrees Fahrenheit (F).
Correction: Adjust the refrigeration temperature so that it is capable of cold-holding the stored foods <= 41 F.
- Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
Observation: The nonfood-contact surfaces of the following equipment were observed soiled with an accumulation of dust, dirt, food residue, and/or other debris: ventilation hood filters, all kitchen equipment, refrigeration compartments, and the refrigeration door handles.
Correction: Maintain nonfood-contact surfaces of equipment clean.

Correction: Clean the kitchen floor, walls, and door as often as necessary to prevent the build-up of grime and debris.

- Sanitizing Solutions, Testing Devices
Observation: There is chlorine no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
Correction: Obtain a chlorine test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.
- Warewashing - Mechanical Warewashing Equipment, Wash Solution Temperature
Observation: The wash temperature of the low temperature chemical sanitizing ware wash machine is not meeting the manufacturer's specifications. The minimum temperature is 120ºF and the actual temperature is 91 F.
Correction: Dishwasher technician notified and will set the wash temperature to 120 F.
- Physical Facilities - Cleaning Frequency and Restrictions
Observation: The floor located under the fryers was noted in need of cleaning.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

- Temperature Measuring Devices; Ambient Air and Water
Observation: The ambient (air/water) temperature measuring device (degrees F) located in the 2 dr prep unit is not accurate. Thermometer is stuck on 60 F. Replace with working thermometer for accurate monitoring.
Correction: Provide a temperature measuring device that is accurate within +/- 3 degrees Fahrenheit in the intended range of use.

Correction: Maintain nonfood-contact surfaces of equipment clean.

Observation: Observed soil / food debris inside utensil holder where clean utensils are stored. CFM put utensils and holder through the dishmachine.
Correction: Maintain separation of soiled and clean tableware to prevent contamination of clean items. Clean and sanitize any contaminated tableware prior to use.

Need to post a notice informing customers that they can review the latest health department inspection upon request. Need to replace the dial probe thermometers with digital probe thermometers. Reviewed employee food allergen training with the owner. Observed the temperature display for the 1 door dairy box is inaccurate, need to place a thermometer in the unit's compartment. A new cutting board is on order to replace the current cutting board. The ventilation hood/filters are scheduled to be professionally cleaned on 2/6/14. A clean and well organized kitchen.
