Observation: A written Employee Health Policy was not available to be seen.
Correction: Informational material was given to the Person in Charge regarding establishing an Employee Health Policy.

Observation:
Found upper section of Make Unit on the side up against the hot holding unit cold holding sliced cheese at 46 F.
Correction: Relocate sliced cheese and any potentially hazardous foods to a colder section of the unit, away from the heat so that these foods can be held at 41 F or lower.

Observation: The prepared ready-to-eat (RTE)/cooked foods inside the W-in cooler were not properly dated for disposition.
Correction: Be more consistent, mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days.

Observation: The staff washroom door was not provided with a self-closure device.
Correction: Provide a self-closure device for the washroom door.

Observation: There was an opening through the threshold of the garbage room exit door that could serve as a rodent entry point.
Correction: Provide a weather-stripping material at the exit door to seal the opening and prevent rodents from gaining access to the area.

Correction: Replace burnt out light bulbs.
