  • CLEAN AND MAINTAIN FLOOR IN AREA WHERE SODA BOX SYSTEM IS LOCATED.
  • CLEAN AND MAINTAIN INTERIOR OF ICE SCOOP STORAGE CONTAINER CONNECTED TO ICE MACHINE.
  • CLEAN AND MAINTAIN WALL IN DRY STORAGE AREA.
  • CLEAN AND MAINTAIN WALL NEAR REAR FOOD PREP SINK.
  • LIDS WERE CLOSED ON EXTERIOR DUMPSTERS DURING INSPECTION.
  • MAINTAIN EMPLOYEE ILLNESS LOG. LOG WAS NOT FOUND DURING INSPECTION. AS DISCUSSED, AN ILLNESS LOG WILL BE MAILED WITH REPORT.
  • OBSERVED SOME EMPLOYEES ENGAGING IN FOOD PREPARATION AND NOT WEARING EFFECTIVE HAIR RESTRAINT (IE. HAT, HAIR NET, ETC.).
  • POST STATE OF MN CERTIFIED FOOD MANAGER CERTIFICATE WITHIN ESTABLISHMENT. NO CURRENT STATE OF MN CERTIFICATE OBSERVED POSTED WITHIN ESTABLISHMENT. APPLICATION IS ATTACHED IF NEEDED.
  • PROVIDE AN APPROVED BACKFLOW PREVENTION DEVICE WITH PRESSURE BLEEDER FOR MOP SINK THREADED HOSE BIBB THAT'S UNDER CONTINUOUS PRESSURE. SEE ATTACHED DIAGRAM. WYE CONNECTIONS ARE PROHIBITED.
  • PROVIDE CEILING PANELS IN PLACE THROUGHOUT KITCHEN. OBSERVED MULTIPLE PANELS NOT SECURE WITHIN CEILING FRAME REPORTEDLY DUE TO ALLOWING WARM AIR INTO CEILING AREA TO PREVENT PIPES FROM FREEZING. INSULATION SHOULD BE PROVIDED.
  • REPAIR LEAKING PIPE FOR HANDWASHING SINK NEAR COOK LINE.
  • REPAIR LOOSE COVED TILE AND PROVIDE GROUT WHERE MISSING ALONG WALL NEAR COOK LINE HANDWASHING SINK.
  • REPAIR LOOSE FRP WALL PANELING IN REAR DRY STORAGE AREA NEAR BASE QUARRY TILE.
