There is a leak in the gas fryer, noted drippage on the floor. A repairman is coming out on Saturday to repair the weld on it.
Violation 45 U: 4-501.11 Good Repair, Proper Adjustment, and Operation.
(A) EQUIPMENT shall be maintained in a state of repair and condition that meets the requirements specified under Parts 4-1 and 4-2.
(B) EQUIPMENT components such as doors, seals, hinges, fasteners, and kick plates shall be
kept intact, tight, and adjusted in accordance with manufacturer’s specifications.
(C) Cutting or piercing parts of can openers shall be kept sharp to minimize the creation of
metal fragments that can contaminate FOOD when the container is opened.
(D) EQUIPMENT shall be used and operated in accordance with the manufacturers’ instructions
and as listed and tested unless APPROVED by the REGULATORY AUTHORITY.
The pipe under the 4 compartment is leaking, replace the o ring seal.
Violation 49.1 A: 5201.11 Plumbing System Constructed and Repaired with Approved Materials. Water faucets must not leak and must be repaired
in compliance with the state of Wisconsin Plumbing Code.
The ladies toilet room sink is chipping, repair or replace the sink.
Violation 51 P: 8-201.17 Toilet Maintenance.
Public toilet rooms shall:
(A) Have a supply of toilet tissue available at each toilet; and
(B) Be in clean and good repair.
The hood filters are soiled, clean and maintain.
Violation 54 F: 6-501.14 Cleaning Ventilation Systems, Nuisance and Discharge Prohibition.
(A) Intake and exhaust air ducts shall be cleaned and filters changed so they are not a
source of contamination by dust, dirt, and other materials.
(B) If vented to the outside, ventilation systems may not create a public health HAZARD or
nuisance or unLAWful discharge.
O.K. to release license pending approval from building inspector. They close on Saturday, and a contractor will come in on Monday to make
some changes to establishment, want to open on 2-24-10 depending if construction is done. Call me at 262-896-8316 when work is completed so I
can do a walk through.

The owner has passed her Serv Safe exam but has not received her certification back from the State yet.

**CDC Risk Factor Violation 07 A**: 3-301.11 Preventing Contamination from Hands.C
• Observed food employees handling ready-to-eat food with bare hands. Violation corrected during inspection (gloves were used). A handout is
also being provided regarding bare hand contact.
(A) FOOD EMPLOYEES shall wash their hands as specified under § 2-301.12.
(B) FOOD EMPLOYEES shall minimize bare hand and arm contact with exposed FOOD that is not in a READY-TO-EAT form.
(C) Except when washing fruits and vegetables as specified under § 3-302.15 or as specified
in ¶ (D), FOOD EMPLOYEES may not contact exposed, READY-TO-EAT FOOD with their bare hands and shall use suitable UTENSILS such as deli tissue,
spatulas, tongs, single-use
gloves or dispensing EQUIPMENT.
Violation 46 H: 4-501.116 Warewashing Equipment, Determining Chemical Sanitizer Concentration.
• Test strips are needed to check the concentration of the sanitizer.
Concentration of the SANITIZING solution shall be accurately determined by using a test kit or
other device.
Violation 43 F: 4-904.11 Kitchenware and Tableware.
• Silverware are noted in containers in a way that contamination can occur.
(A) SINGLE-SERVICE and SINGLE-USE ARTICLES and cleaned and SANITIZED UTENSILS shall be handled, displayed, and dispensed so that contamination
of FOOD- and lip-contact surfaces is prevented.
(B) Knives, forks, and spoons that are not prewrapped shall be presented so that only the
handles are touched by EMPLOYEES and by CONSUMERS if CONSUMER self-service is provided.
Nora has the ServSafe certified manager course completed. She sent in to the state but could not locate it during inspection. She stated that
once it is located it will be faxed over to our office.

4 compartment sink using quat sanitizer (not set up), low temperature dish machine using Sanitizer ES chlorine sanitizer at 100 ppm and hand
wash sink (one in the kitchen and one in the front).
