    Observation: Employee observed not washing hands when going back to food handling after other duties and also using towel hanging from belt to wipe hands in lieu of proper handwashing.
    Correction: Before working with food, clean equipment, utensils, and single-use or single service articles food employees shall properly wash their hands.
  • CONTROLLING PESTS - TRAPPING DEVICES
    Observation: Many fruit flies were noted at inspection.
    Correction: Eliminate pests or insects from food establishment. Note - Establishment has already begun work with Batzner Pest Control to correct this issue.
  • EQUIPMENT AND UTENSILS - GOOD REPAIR
    Observation: Cracked / damaged containers including the breading tank and lexan lids were noted at inspection.
    Correction: Repair equipment to good condition or remove from premise.
  • NON-FOOD CONTACT SURFACES - CLEAN (repeated violation)
    Observation: Non-food contact surfaces of equipment such as the mixer and soda dispenser have a build up of food debris.
    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.

    Observation: There are leaks in the faucets at the mop sink and warewashing sink.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing.

    Observation: Employee noted going to hand sink to get paper towels to wipe hands but not washing their hands and using sanitizer bucket and wiping cloth to clean hands. This may be due to reluctance to use the hand sinks due to the water spraying on to anyone who tries to properly wash their hands. See comments regarding hand sinks. At recheck as noted in comments hand sinks still spray water during use and cannot be used to properly wash hands without wetting both the floor and the person trying to wash their hands.
    Correction: Before working with food, clean equipment, utensils, and single-use or single service articles food employees shall properly wash their hands.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR (repeated violation)
    Observation: Hand sinks are splashing out water on anyone trying to use them causing a reluctance to wash hands and a ponding of water on adjacent floors. As noted in the comment section, this has been an ongoing issue and has been ignored. Appropriate ongoing violation and reinspection fees will be charged until this is corrected. At recheck when trying to wash hands at back hand sink I was wet down to / including my feet.
    Correction: Repair / replace hand sinks to provide a proper hands free flow that employees are able to use.

    Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
  • FOOD CONTACT SURFACES - NON TCS/PHF -IMPROPER CLEANING
    Observation: There is a build up of slime mold in the ice chute of the self service soda machine.
    Correction: Clean utensil and equipment not contacting PHS/TSC food any time contamination occurs, at least every 24 hours, or at a frequency according to manufacturer specifications.Clean and sanitize the ice bin / chute of the soda machine.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
    Observation: The back hand sink drain is slow and the drain for the discharge of the adjacent ice machine is overflowing.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR (repeated violation)

    Observation: Tools, old electrical receptacles, repair equipment, etc are stored on top of the Blodgett convection oven.
    Correction: Items that are unnecessary to the operation of the food establishment shall be removed from the premise - remove these items and clean the tops of the ovens.

    Observation: The back hand sink is splashing water out to create a ponding effect on the floor and no one is using it. At recheck Management indicated that repairs had been scheduled to avoid fees.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing. This has been an ongoing issue - appropriate fees will be charged for ongoing violations if this is not properly repaired or replaced.

  • Critical: WHEN TO WASH
    Observation: Employees are not washing hands as required - until this was brought to the attention of the manager, I only saw one person wash their hands although several employees were dealing with foods including raw chicken.
    Correction: Before working with food, clean equipment, utensils, and single-use or single service articles food employees shall properly wash their hands.
  • Critical: GLOVES, USE LIMITATION - SINGLE-USE GLOVES
    Observation: Single-use gloves are not always discarded between uses, used for multiple projects, and hands are not washed when donning new gloves.
    Correction: Single-use gloves shall be discarded between tasks such as working with ready-to-eat foods or raw animal foods, when they become damaged or soiled or when interruptions occur in the operation.
  • Critical: PHF/TCS, HOT HOLDING
    Observation: Foods such as chicken in the buffet line are not being held above 134 degrees. Establishment is using time as a quality control but not as a health control as they are breaking down chicken pieces for deboning and later use for such thihngs as pot pie.
    Correction: Maintain hot potentially hazardous foods at or above 135°F.
  • Critical: FOOD CONTACT SURFACES - SANITIZED BEFORE USE AFTER CLEANING
    Observation: There was no sanitizer residual in the sanitize water in the warewash sink. It was drained and refilled and adjusted to proper level of 200 ppm Quat.
    Correction: Clean and sanitize all utensils and equipment before contact with food. Check sanitizer levels frequently with the available test strips to ensure sanitizer is available and at proper concentration.
  • PERSON IN CHARGE - DUTIES
    Observation: The Person in Charge (PIC) is not ensuring compliance with the food code as shown by deficiencies noted during the inspection.
    Correction: The person in charge shall ensure compliance with the food code and that food and drink are handled in a safe manner.
  • CERTIFIED FOOD MANAGER – REQUIREMENT
    Observation: There is no Wisconsin Certified Food Manager for this establishment. Manager indicated three persons should be certified - Maria Nuno, Rosa Lopez, and Santiago Soto. None of these were in the data base and upon contacting the state, none of them are Wisconsin Certified Food Managers
    Correction: Provide a Wisconsin Certified Food Manager and post certification in the food establishment BEFORE January 18, 2014. Appropriate fees wil be charged for operating without the required Certified Food Manager and this was an issue in the past.
  • WIPING CLOTHS - USE LIMITATION - SANITIZING SOLUTIONS
    Observation: Wiping cloth solution in the fryer area is dirty.
    Correction: Maintain wiping cloth sanitizer solution clean and free from visible debris.
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
    Observation: The back hand sink is splashing water out to create a ponding effect on the floor and no one is using it.
    Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing. This has been an ongoing issue - appropriate fees will be charged for ongoing violations if this is not properly repaired or replaced.
  • GARBAGE/REFUSE - STORAGE REFUSE, RECYCABLES, AND RETURNABLES
    Observation: There were numerous broken down boxes and bags of garbege in the kitchen at start of inspection.
    Correction: Take garbage & recycleables to the dumpster - do not store in kitchen.
  • HANDWASHING SIGNAGE
    Observation: No handwashing signage provided at back handwashing sink.
    Correction: Provide handwashing signage at all handwashing sinks.
  • DISPOSABLE TOWELS, WASTE RECEPTACLE
    Observation: No waste receptacle provided at back handwashing sink.
    Correction: Provide a waste receptacle at handwashing sinks that use single-use paper toweling.
  • CLEANING, FREQUENCY AND RESTRICTIONS
    Observation: There is a large amount of standing water and floating food debris by the back hand sink.
    Correction: Until proper repairs are made to the sink, continually mop to eliminate standing water and debris.

The place is disgusting! I was told by the manager after waiting an hour for my food not to come back again because I complained about the staff! I ordered a $20 bucket the chicken was hard as could be it was like eating beef jerky! I literally felt afraid of the manager that was there. I can't believe that they can continue to keep a store like this open with garbage laying all over the floor on top of the tables not using gloves when they're serving food and putting an end containers. When I received my food they were only half full didn't put items in had to check it and they gave me grief about it also when waiting waited 45 minutes just to get my order looked at for families walk in and walk out because of the wait. I will never ever go back there again I drove 10 hours from tennesee to Wisconsin wanted to get something to eat only the wait an hour to get chicken and to see nastiness! And the manager to give me a hard time and tell me never to come back to his store again I have never been so appalled in my life!
