Observation: Some of the cheeses using time as a public health control for 4-hours is not properly marked or identified with a discard time.
Correction: Food items shall be marked or otherwise identfied with a time that is 4-hours past the point in time when the food was removed from temperature control. Discard food item.

Observation: Employee noted working in food preparation/cooking area without a hair restraint.
Correction: Provide employees working in food preparation and cooking areas with effective hair restraint (hat, hair net, beard restraint, etc.).

Observation: Wiping cloths used for wiping counters not put into sanitizer buckets.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.

Observation: There is stainless steel on the top of the ice machine that has separated as pointed out at inspection leaving a gap that cannot be properly cleaned and sanitized.
Correction: Secure this so that the surface can be properly cleaned and sanitized.

Observation: There are water leaks at the mop sink and the warewashing sink..
Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing.

Observation: Employees drink was in the #6 cooler on top of for sale foods.

Observation: Employee clothing was hanging from shelving by eyewash station.
Correction: Provide lockers for employees and instruct employees to use lockers for storage of their personal clothing and possessions.
