HANDWASHING FACILITIES - NUMBERS AND CAPACITIES (repeated violation) Observation: Currently this facility has a 2-compartment sink, which they are using as a handwash sink and warewash sink.
Correction: Handwash sink cannot be used as one side of a 2-compartment warewash sink. A dishwasher must be installed in order to utilize one of the two sinks as a handwash sink or a 3-compartment sink and separate handsink must be installed.
CERTIFIED FOOD MANAGER – REQUIREMENT (repeated violation) Observation: There is no Wisconsin Certified Food Manager for this establishment. Jacob Banas has attended an approved food manager's course. Send to FSRL a copy of application, certification, and $10.00.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days. If not recieved within 90 days, a $150.00 fee will be assessed this facility.
WAREWASHING - SINK COMPARTMENT REQUIREMENTS - TWO COMPARTMENT PROHIBITIONS Observation: Two compartment sink is being used in a continuous warewashing operation. Bleach is currently being used as a sanitizer in this two compartment sink.
Correction: A two compartment sink shall only be used in a batch warewashing operation. Provide a three compoartment sink or commercial dish machine. Additionally, when using two compartment sinks, a detergent sanitizer is required. Bleach is strictly a sanitizer only and cannot be utilized for warewashing in a two compartment sink. However, two compartment sink batch washing is not approved for use at this restaurant as continuous dishwashing is necessary at this facility due to the amount of dishes used and the nature of the way the food is prepared and served.
SURFACE CHARACTERISTICS - INDOOR AREAS Observation: Window has unsealed wood surrounding air conditioner.
Correction: Repair area or replace with approved materials. Walls, floors and ceilings shall be smooth, durable and easily cleanable. If located in areas subject to moisture they shall also be non-absorbent.
TOILET FACILITIES - ENCLOSED Observation: Toilet room in consumer/bar area is not provided with a tight fitting self-closing door.
Correction: Provide a self- closing door mechanism on bathroom door to provide a tight fitting self closing door.

Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
HANDWASHING FACILITIES - NUMBERS AND CAPACITIES Observation: Handwashing sink not provided in kitchen. There is an area in the bar.
Correction: Install an approved handwashing sink before operating as a restaurant. The current warewash sink may again be used if you replace the dishmachine.
