    Observation: The jelly is prepared in a private home.
    Correction: Discard food from unapproved sources.
  • OPEN DRINK CONTAINER
    Observation: Coffee cup without cover was observed in food preparation area.
    Correction: Provide approved beverage container in food preparation area.
  • PACKAGED AND UNPACKAGED FOOD - COVERED CONTAINERS OR WRAPPING
    Observation: There are loaves of bread in the walk-in cooler that are uncovered.
    Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.
  • NON-FOOD CONTACT SURFACES - CLEANING FREQUENCY
    Observation: All three freezers have frost build up. There is grease and duct on the underside of the flat top and the board on the front.
    Correction: Clean all equipment on a routine basis to prevent accumulation of soil residues.
  • FLOORS, WALLS AND CEILINGS - CLEANABILITY
    Observation: The plywood behind the fryer is exposed around the bottom and left edge. The ceiling tiles are acoustical rather than smooth and non-porous. There are floor tiles missing under the fryer. The edge of the walk-in cooler is lifting causing a gap.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.

    Observation: The coving in the corner under the dish machine is missing.
    Correction: Provide base coving.

    Observation: Several food items (gravy, potatoes, meatballs, fish batter) in coolers are not date marked.
    Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.

    Observation: Spaghetti sauce in walk in cooler not date marked and found to have mold growing on the inside of container.
    Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.

    Observation: Employee noted working in food preparation/cooking area without a hair restraint.

    Observation: The fish and beef stored on the floor in the walk in freezer.
    Correction: Store all food items 6 inches above the floor.
  • INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS (corrected on site)
    Observation: The exterior garage door in the back storage room was not closed at the start of the inspection but was closed during the inspection.
    Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.

    Observation: True dessert cooler is not equipped with a thermometer.

    Observation: Inside of microwave is soiled.
    Correction: Clean and maintain microwave.

    Observation: The booths with holes and rips need to repaired/replaced.

    Observation: The following areas need to be cleaned:
    -wall surface under coffee cup shelf
    -clean shelves under coffee pots under of coffee splatter
    -floors under grill, stoves, and fryers of grease, food product and other debris

    Correction: Clean the physical facilities at a frequency necessary to keep them clean.
