Critical: FOOD CONTACT SURFACES - WAREWASHING EQUIPMENT, CHEMICAL, TEMPERATURE, Ph, CONCENTRATION AND HARDNESS (corrected on site) Observation: No chlorine sanitizer was detected in the dishwasher final rinse. The liquid in the supply container was found to be of low concentration.
Correction: Provide training to employees on proper use and concentration of sanitizer. A replacement container of sanitizer was installed and primed and then the final rinsed tested 50 ppm.
CONSUMER ADVISORY Observation: The consumer advisory on the menu is not complete.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition. Add the disclosure as discussed.

Critical: PHF/TCS, COLD HOLDING Observation: Potatoes cleaned and foiled for baked potatoes are sitting on the counter at 60°F since 3/22/15. Potatoes are set out on cart until they are needed, then baked in the oven.
CORRECTED DURING INSPECTION: Potatoes were disposed of.
Correction: Maintain cold potentially hazardous foods at or below 41°F

Correction: Discard food out of temperature control. Equipment for vending or dispensing food out of temperature control shall be specifically designed and equipped to maintain the commercial sterility of aseptically packaged food in a homogenous liquid form for a specified duration from the time of opening of the packaging within the equipment.
