Observation: No chlorine sanitizer was detected in the dishwasher final rinse. The liquid in the supply container was found to be of low concentration.
Correction: Provide training to employees on proper use and concentration of sanitizer. A replacement container of sanitizer was installed and primed and then the final rinsed tested 50 ppm.

Observation: The consumer advisory on the menu is not complete.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition. Add the disclosure as discussed.

Observation: A chlorine test kit is not available for checking sanitizer concentrations at the mechanical dishwasher and quat test strips are not available at the bar sinks.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.

Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment. Fernando took and passed a class in April but did not send his application to Madison.
Correction: Provide & post an original State of Wisconsin food manager's certificate. An application is provided with this report.
- EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS
Observation: The domestic refrigerator in the kitchen is not ANSI certified or approved by the department and is used to store meats and other potentially hazardous foods.
Correction: Unapproved equipment shall be removed from food service.

Observation: Lights located in the walk-in cooler are not shatter resistant.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.

Observation: Potatoes cleaned and foiled for baked potatoes are sitting on the counter at 60°F since 3/22/15. Potatoes are set out on cart until they are needed, then baked in the oven.
CORRECTED DURING INSPECTION: Potatoes were disposed of.
Correction: Maintain cold potentially hazardous foods at or below 41°F

Observation: Whirlpool upright refrigerator is not designed to maintain the commercial sterility of the aseptically packaged food: Mozzarella cheese, lettuce, and other salad bar items.
(CORRECTED DURING INSPECTION) Employee removed these items and placed them in either the upright Traulsen cooler or the walk-in cooler.)
Correction: Discard food out of temperature control. Equipment for vending or dispensing food out of temperature control shall be specifically designed and equipped to maintain the commercial sterility of aseptically packaged food in a homogenous liquid form for a specified duration from the time of opening of the packaging within the equipment.
