Critical: COOLING Observation: Rice made this morning (3-1-16) apox 10:30 is at 128'F in plastic pan in the walk-in cooler. Talked with cook about adjusting his cooling procedures to ensure adequate cooling time. He will begin to put rice into shallow pans to properly cool under refrigeration and then transfer to plastic pan for storage in walk in.
Note: Container of pork fat / pork juices stored on counter for use later. Any lard/fat that is used for cooking or food prep that is to be reused, must be properly cooled and stored under refrigeration for 7 day holding period
Correction: Adjust method or procedures to cool foods from 135°F to 70°F in the first 2-hours and from 70°F to 41°F in an additional 4-hours or if prepared or received at ambient temperatures cool to 41°F in 4-hours.
Critical: RTE/PHF-TCS PROPER DATE MARKING Observation: Most foods are marked with dates. Several foods in various refrigerators are not date marked Date marking not consistent.
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR Observation: Several food items in walk-in freezer stored on floor. Please store items off the floor.
Correction: Store all food items 6 inches above the floor.

USING A HANDWASHING SINK - ACCESSIBLE Observation: Handwashing sink observed to have a cardboard box on top of it and also a trash can in front of it during inspection and is unavailable for proper handwashing.
Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing.
HANDWASHING CLEANSER AVAILABILITY (repeated violation) Observation: Soap not available at bar sinks for employee handwashing.
Correction: Provide hand soap at bar sinks to facilitate proper handwashing.
HAND DRYING PROVISION Observation: No single-use toweling or other hand drying device available at kitchen handwashing sink for hand drying.
Correction: Provide single-use toweling or other approved devices at kitchen handwashing sink to facilitate proper handwashing.
TOXIC SUBSTANCES - IDENTIFYING INFORMATION, PROMINENCE - COMMON NAME-WORKING CONTAINER Observation: 1. Bottle with bleach behind bar is not labeled. 2. Spray bottle of sanitizer (for tables) is not labeled.
Correction: Label all bottles of bleach or sanitizer with contents.
IN-USE UTENSILS, BETWEEN USE STORAGE (repeated violation) Observation: Ice scoop inside of the ice bin in the server area was laying with handle in direct contact with ice.
Correction: Keep ice scoop stored with the handle up and out of the drink ice.
EQUIPMENT- COMPARTMENTS - DRAINAGE Observation: The drain for the first sink of the 3 compartment sink in the dish room is missing.
Correction: Repair drain for sink.

Critical: CLEAN CONDITION - HANDS AND ARMS Observation: See other violation relating to lack of soap, towels, at hand wash sinks.
Correction: Due to the lack of soap and/or towels at 3 handwash sinks in the service area, kitchen and dishroom, it is very difficult for food handlers to properly wash their hands. Provide soap and paper towels at all of these sinks at all times and instruct employees on proper handwashing procedures..
Critical: DATE MARKING - DISPOSITION Observation: Most items in coolers are marked woth dates. However there are still many items that are not date marked properly. I also found some foods in the cooler that were past the 7 day use by date. (Pork mole)..
Correction: Date mark ALL ready to eat foods that are prepared and then stored in the cooler. Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.
HANDWASHING CLEANSER AVAILABILITY Observation: Handwash sink in front server area does not have soap. Handwash sink in kitchen does not have paper towels in dispenser. Handwash sink in dishroom does not have soap and paper towels is jammed.
Correction: Provide hand soap and paper towels at ALL handwashing sinks to facilitate proper handwashing.
IN-USE UTENSILS, BETWEEN USE STORAGE Observation: Ice scoop at in the ice bin at server station was laying directly on ice....also knife was stored underneath coffee machine. Ice scoop for ice machine in back room is not covered.
Correction: .Always store ice scoops in the ice bins with the handle sticking up and out of the ice. Do not store knives underneath coffee machine. Store ice scoop for ice machione so it is covered (plastic bag).
EQUIPMENT AND UTENSILS - MATERIAL FOR CONSTRUCTION AND REPAIR Observation: There is mold/mildew on the caulk near the prewash sink faucet in the dish room.
Correction: Clean caulk....if mold cannot be cleaned, remove the caulk and recaulk the area between the wall and the backsplah of the sink.
