    Observation: The pizza prep cooler is maintaining foods at 43F.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. The operator adjusted the thermostat at the time of this inspection.
  • Critical: FOOD-CONTACT SURFACES AND UTENSILS
    Observation: The dishwashing machine is not bringing the plate surfaces to 160F (as shown by a thermolabel).
    Correction: The dishwashing machine must bring the plate surface to 160F. Sanitize equipment and utensils in the 4 compartment sink until the machine meets this requirement.
  • Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED (repeated violation)
    Observation: No approved backflow preventor is installed on the mop sink hose bibb. (a hose is attached)
    Correction: Provide a hose bibb backflow preventor at the faucet before the hose is attached. Correct By: 04-Apr-2016

    Observation: There is no Wisconsin Certified Food Manager for this establishment.
    Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment. The operator stated that the cook is signed up to take the technical college course in Sturgeon Bay in early April. Correct By: 31-May-2016
  • WAREWASHING MACHINES - OPERATING INSTRUCTIONS
    Observation: The warewashing machine is not operated according to manufacturers directions.
    Correction: Operate the warewashing machine consistent with the machine’s data plate and other manufacturer’s instructions. This machine requires the wash water to be 150F minimum. Low wash water temperature may cause problem with meeting the sanitization temperature requirement. Correct By: 11-Mar-2016
  • LIGHT BULBS - PROTECTIVE SHIELDING (repeated violation)
    Observation: Lights are missing the end cap. Broken glass could spill out the end of the plastic tubes.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs. The operator stated that new fixtures will be installed this spring. Correct By: 31-May-2016

    Observation: Ice making water is coming into contact with moldy surfaces inside of the ice machine.
    Correction: Clean and sanitize the ice machine. The machine must be disassembled, cleaned, and sanitized at the frequency that precludes mold. Correct By: 24-Mar-2015
  • Critical: FOOD CONTACT WITH EQUIPMENT AND UTENSILS
    Observation: Soda beverages are coming into contact with slime mold in the soda gun heads and their drip cups.
    Correction: Clean and sanitize the soda gun heads and drip cups at a frequency that precludes slime mold. Use a flashlight to determine if they are adequately cleaned. Correct By: 24-Mar-2015

    Observation: The food prep cooler is not keeping potentially hazardous foods at or below 41F. Multiple food items in the upper unit and lower unit were found at 48F-52F.
    Correction: Maintain cold potentially hazardous foods at or below 41°F. The operator removed all of the potentially hazardous food items from this cooler and placed them into the walk in cooler at this inspection. The operator contact a refrigeration technician and the technician is scheduled for a visit today.
  • Critical: FOOD CONTACT SURFACES - SANITIZED BEFORE USE AFTER CLEANING (corrected on site)
    Observation: The bar's glasswasher did not provide a final rinse chlorine sanitizer concentration of 100 ppm. The concentration on arriving was 0 ppm.
    Correction: The bar's glasswasher must deliver a 50 ppm final rinse according to the manufacturer's data plate. The operator replaced the sanitizer jug and primed the line. The concentration found after draining and refilling and cycling the machine was 50 ppm.
  • Critical: PLUMBING - BACKFLOW PREVENTION DEVICE - WHEN REQUIRED
    Observation: A hose bibb backflow preventor is not installed at the mop sink faucet.
    Correction: Provide a hose bibb backflow preventor at this faucet connection. Correct By: 30-Mar-2015
  • CONSUMER ADVISORY
    Observation: The restaurant occasionally serves hamburgers cooked to the consumer's liking. The consumer advisory does not disclose that hamburgers cooked rare, medium rare, or pink in the middle may be undercooked.
    Correction: A written consumer advisory that both discloses what food items are undercooked, and reminds consumers that eating the undercooked item is an increased health risk, is required. For example, place an asterisk next to the Burger section of the menu that leads to a footnote that states "Hamburgers requested rare, medium rare, or pink in the middle may be undercooked. Whether dining out or preparing food at home, consuming undercooked meats, fish, seafood, poultry, or eggs, may increase your risk of foodeborne illness.
  • FOOD CONTACT SURFACES - SOILED
    Observation: The can opener has an accumulation of old food debris.
    Correction: Clean and sanitize the can opener at least every four hours if it has been used. Correct By: 23-Mar-2015

    Observation: A knife was stored in the crevice between two tables. The crevice is not clean.
    Correction: Discontinue storing knives in the crevices between tables. Store the knives on a clean surface or in some other manner that prevents the utensil from contamination. Correct By: 23-Mar-2015
  • LIGHT BULBS - PROTECTIVE SHIELDING (repeated violation)
    Observation: Lights throughout the kitchen do not have the end caps that would hold in glass particles if the bulb should be shattered. The lights in the walk in cooler are not shatter-resistant bulbs.
    Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs. Make sure the caps are provided on the fluorescent fixtures. Correct By: 30-Mar-2015

    Observation: Some of the bars particle board shelving is in poor repair.
    Correction: Maintain the physical facilities so they are in good repair at all times.

    Observation: Food is stored on the floor in the walk in freezer.
    Correction: Store all food items 6 inches above the floor.
  • EQUIPMENT AND UTENSILS - BEVERAGE TUBING - SEPARATION
    Observation: Beverage tubing or cold-plate located in ice bin is in direct contact with ice.
    Correction: Remove beverage tubing or cold-plates from contact with stored ice.
  • FLOORS, WALLS AND CEILINGS - CLEANABILITY
    Observation: The ceiling in the diswashing area has some holes and needs repair.
    Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
