Critical: SEWAGE - BACKFLOW PREVENTION Observation: There is an air break rather than an air gap on both ice bins at the side bar.
Correction: Design the sewage system in a manner that would preclude a direct connection between the sewage system and the drain from which food is placed. Remove the direct connection.
CLEANING, FREQUENCY AND RESTRICTIONS Observation: There is debris and a dark substance on the floor under the cook line.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

FLOOR AND WALL JUNCTURES, COVED, AND ENCLOSED OR SEALED Observation: There are a few coving tiles under the food prep sink that are no longer attached to the wall. There are two coving tiles under the 4 compartment kitchen sink that are no longer attached to the wall. The coving tiles in the back room by the walk-in cooler are pulling away from the wall causing a gap between the tiles and the wall. There are broken/missing floor tiles by the back door and under the shelving unit by the walk-in cooler.
Correction: Repair/replace the base coving and floor tiles. Correct By: 16-Sep-2014
INSECTS AND RODENTS - OUTER OPENINGS PROTECTED Observation: There is a gap under the back door which could allow the entrance of insects or rodents.
Correction: Repair this gap so that entrance by insects or rodents into the food establishment is prevented. Correct By: 16-Sep-2014
