Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
CONSUMER ADVISORY (repeated violation) Observation: No consumer advisory provided on menu.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
FOOD PREPARATION SINK Observation: The food prep sink is not being used for food prep. Today burrito meat was being strained in the pre-wash sink. USe the separate food prep sink for food prep exclusively.
Correction: A food preparation sink shall be provided for significant food preparation or in instances where food preparation is occurring below the flood level rim of a sink compartment such as thawing or vegetable washing.

FOOD TEMPERATURE MEASURING DEVICES - PROVIDED Observation: Various coolers had no thermometer in them. Pizza prep and home fridge.
Correction: Provide thermometers that are easily accessible and capable of taking temperatures in food.
WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES Observation: A chlorine test kit is available for checking sanitizer concentrations but the cap has been lost and strips are no longer usable since they have been wet.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
CERTIFIED FOOD MANAGER – REQUIREMENT Observation: There is no Wisconsin Certified Food Manager for this establishment. Al NEro's food certificate expired 5/14.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 3 months.
EQUIPMENT AND UTENSILS CONFORM TO ANSI STANDARDS Observation: Home style frigidaire refregerator is not ANSI certified or approved by the department.
