ASSIGNMENT Observation: There is no designated person in charge (PIC) available at the time of inspection.
Correction: A person in charge shall be provided on premises at all times during hours of operation.
WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES Observation: A test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
USING A HANDWASHING SINK - ACCESSIBLE Observation: Handwashing sink next to the ice machine observed to be with a large Pine Sol bottle during inspection and is unavailable for proper handwashing.
Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing.
CERTIFIED FOOD MANAGER – REQUIREMENT Observation: There is no Wisconsin Certified Food Manager for this establishment. Deb has recently taken the course. An application will be included with this report.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days.
POSTING OF CERTIFICATE Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.

WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER Observation: Wiping cloth used for wiping counters stored on counter.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.
FOOD CONTAMINATION PREVENTED - MISCELLANEOUS CONTAMINATION Observation: Tomatoes and lettuce are subject to potential contamination by being stored in a non-food grade purple container.
Correction: Change methods or procedures to protect foods from contamination.
SINGLE-USE AND SINGLE SERVICE ARTICLES - ORIGINAL CONTAINER Observation: Single use carry out containers not stored in original container are not properly protected on the cookline with food contact surfaces exposed.
Correction: Store single-use articles in their original container or use other effective means to prevent contamination.
PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR Observation: The food prep sink is not maintained in good repair and is leaking into a metal pan on the floor.
The prewash sprayer is laying on the prewash counter.
Correction: The plumbing system shall be maintained in good repair. Repair or replace defective or leaking plumbing.
