Soap was not provided to the front hand sink. Paper towels were not provided to the back room hand sink.
There was a build up of ice in the reach-down freezer.
The wall above the mop sink basin was peeling. The wall was peeling above the three-compartment sink. The wall by the ice machine was soiled.
The lights were not shielded in the walk-in refrigerator.
There were several piece of unused equipment in the back room.
