REMOVE THE ROTTEN WOOD UNDER THE FRONT COUNTER HAND SINK AND RESEAL THIS AREA SO IT IS LIGHT COLORED, SMOOTH AND EASY TO MAINTAIN. WASH, RINSE AND SANITIZE THE FAN GUARDS IN THE WALK-IN COOLER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. SEAL THE CRACK IN THE FLOOR BY THE REAR PARKING LOT DOOR AND FILL IN THE GAPS IN THE BASEBOARD TILES TO THE RIGHT OF THE OVENS.
The walls and ceilings shall be in good repair and easily cleaned. CLEAN AND/OR REPAINT THE REAR KITCHEN WALLS WHERE THEY ARE SOILED.

THE REAR WALK IN COOLER HAS A MISSING TILE AND SOME BUILDUP BY THE DOOR AND THE FRONT WALK IN COOLER HAS A GAP ALONG THE WEST WALL BASEBOARD, THE BASE BOARD BEHIND THE WHITE FIRE SYSTEM BOTTLE HAS A GAP, CLEAN AND MAINTAIN THE FLOORS AND BASEBOARDS SO THEY ARE SMOOTH AND SANITARY FROM CORNER TO CORNER.

All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.MUST DEEP CLEAN INTERIOR OF ALL DEEP FRYERS ON PREMISES.MUST CLEAN EXTERIOR OF GREASE TRAP BOX TO REMOVE EXCESS GREASE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.MUST REPAINT RUSTY EXTERIOR OF GREASE TRAP BOX UNDER 3 COMP SINK.

MUST REPAIR LEAKY PIPE UNDER 3COMP SINK IN REAR PREP AREA AS WELL AS THE LEAKY HOT WATER HANDLE BASE.MUST CHANGE GREASY EXHAUST HOOD VENTS AT REAR AREA.
MUST DETAIL CLEAN ALL FLOOR CORNERS TO REMOVE GRIME BUILD UP AT FRONT COOKING AREA.MUST REPAIR HOLE AT CEILING IN FRONT OF THE REAR WALK-IN COOLER.
MUST SEAL ALL OPENINGS AT FLOOR IN THE FOLLOWING LOCATIONS: UNDER ICE MACHINE AT FRONT SERVICE COUNTER,IN THE BATHROOM BEHIND THE DOOR,AND BEHIND REAR STOVE SO THEY ARE SMOOTH AND EASILY CLEANABLE.
MUST PROVIDE A SELF-CLOSING DEVICE AT STAFF TOILET ROOM.
