All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily.CLEAN DIRTY FAN GUARDS IN WALK-IN COOLER,COFFEE COUNTER TOP, INTERIOR OF MICROWAVE, STORAGE SHELF IN 2DR FREEZER AND DEFROST AND CLEAN ICE BOX FREEZER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS THROUGHOUT UNDER AND AROUND ALL STORAGE UNITS AND REAR SINKS.
The walls and ceilings shall be in good repair and easily cleaned. REAR SINK AREA OBSERVED TWO HOLES IN THE WALLS NEED REPAIR. ALSO HOLE OUTSIDE WALK-IN COOLER DOOR MADE BY THE HANDLE MUST REPAIR.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. TOILET ROOM DOOR NOT SELF CLOSING. INSTRUCTED TO PROVIDE DOOR CHECK TO MAKE DOOR SELF CLOSING.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. OBSERVED NO DRAIN BOARD INSTRUCTED TO PROVIDE.

OBSERVED ALL LOWER WALLS THRU-OUT PREMISES AND BASEBOARDS NOT CLEAN, EXCESSIVE DEBRIS. INSTRUCTED TO CLEAN ALL WALLS/MAINTAIN.

FOUND PLASTIC SINGLE SERVE UTENSILS IMPROPERLY STORED IN CARDBOARD BOX. MUST PROPERLY STORE UPRIGHT IN CLEAN PLASTIC CONTAINER.
MUST REMOVE ALL CARDBOARD AND NEWSPAPER FROM SERVICE TABLE WHERE GROUND BEEF IS SERVED. ALSO MUST REMOVE DIRTY STORAGE MILK CRATES STORED UNDER SERVICE TABLE. INSTRUCTED TO PROVIDE AN ADEQUATE PREP TABLE FOR STORAGE OF COOKED FOODS. FOUND OPEN BAG OF SUGAR IN CABINET AT COFFEE SERVICE STORAGE AREA. INSTRUCTED TO PROVIDE LARGE STORAGE CONTAINER WITH LID.
OBSERVED STORAGE SHELVES NOT CLEAN BEHIND COUNTER, 3 COMPARTMENT SINK INSTRUCTED TO CLEAN AND MAINTAIN.
OBSERVED DIRTY MOP WATER IN MOP BUCKET. ALSO MOP SINK NOT CLEAN. INSTRUCTED TO DUMP WATER AFTER USE AND PROPERLY STORE MOP UPRIGHT. MUST CLEAN/MAINTAIN MOP SINK. ALSO FOUND REAR STORAGE AREA CLUTTERED(PAINT CANS, ETC, MUST PROPERLY STORE STORAGE ITEMS OFF FLOORS ON PALLET OR SHELVING.
