All food should be properly protected from contamination during storage, preparation, display, service, and transportation. FOOD (ICE) NOT PROTECTED DURING STORAGE INSIDE OF THE ICE MACHINE. OBSERVED BLACK MOLD GROWTH ON INTERIOR CONTACT SURFACES OF THE MACHINE. UNIT TAGGED BY CDPH. MANAGEMENT INSTRUCTED TO HAVE THE UNIT CLEANED AND SANITIZED BEFORE USE. FAX REINSPECTION LETTER TO (312) 746-4240. SERIOUS VIOLATION 7-38-005A.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. REPALCE THE BROKEN FLOOR TILES BEHIND THE BAR. REPAINT THE AREAS OF THE BASEMENT FLOOR WHERE NEEDED.
The walls and ceilings shall be in good repair and easily cleaned. SEAL ANY OPENINGS IN THE WALLS OF THE PREP AREA AND BEHIND THE BAR.
Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated. THE VENTILATION FANS IN BOTH RESTROOMS ARE NOT OPERATIONAL. MUST REPAIR.

The walls and ceilings shall be in good repair and easily cleaned. OBSERVED SECTIONS OF CEILING DRYWALL MISSING ABOVE THE ICE MACHINE. MUST PROVIDE.
