OBSERVED LIVE FRUIT FLIES ON THE WALLS, CEILING AND EQUIPMENT IN THE DINING AREA, FRONT SERVICE AREA AND PREP AREA. APPROXIMATELY 40 LIVE FRUIT FLIES WERE OBSERVED IN THESE AREAS. MUST ELIMINATE OR MINIMIZE THE PEST ACTIVITY. ALSO, PEST CONTROL LOG BOOK IS NOT BEING MAINTAINED. THE LAST INSPECTION REPORT INSIDE OF THE LOG BOOK WAS FROM 9/24/11. MUST PROVIDE A CURRENT PEST CONTROL REPORT AND KEEP IT IN THE LOG BOOK. SERIOUS VIOLATION 7-38-020.
PREVIOUS INSPECTION REPORT SUMMARY FROM 8/18/10 WAS NOT VISIBLE TO CUSTOMERS. MANAGER WAS GIVEN A COPY OF TODAY'S INSPECTION REPORT SUMMARY AND WAS INSTRUCTED TO POST IT IN VIEW OF ALL CUSTOMERS. SERIOUS VIOLATION 7-42-010B.
