Toilet rooms shall be completely enclosed and shall be vented to the outside air or mechanically ventilated.TOILET ROOM DOOR IN BASEMENT MUST BE SELF CLOSING.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines.FOUND REAR DOOR TO PREMISES NOT RODENT-PROOFED.
Adequate and convenient toilet facilities shall be provided. They should be properly designed, maintained, and accessible to employees at all times.ADDITIONAL EXPOSED HANDSINKS ARE NEEDED IN THE KITCHEN AREA,DISHROOM.
The walls and ceilings shall be in good repair and easily cleaned.WALLS AND CEILINGS IN KITCHEN AREA,INSIDE WALK-IN-COOLERS MUST BE SMOOTH,EASILY CLEANABLE.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors.MUST CLEAN,ORGANIZE THE PREMISES,REMOVE ALL CLUTTER.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair.FLOORS AONG WALLS AND IN ALL CORNERS REQUIRE DETAIL CLEANING,INCLUDING WALK-IN-FLOORS.

Found gap under rear receiving door approximately 1/4' mode of entry all accross the bottom of said door.Serious violation 7-38-020.
Remove cluttered conditions in the basement storage rooms due to potential pest harborage,remove pieces of unused kitchen equipments,properly store remaining items 6' off floor/wall for easier cleaning.All cleaning equipment should be stored clean & hung to dry.
Floors along walls and in all corners throughout especially in basement storage areas require a detail cleaning.
Walls and ceilings in the south dry storage room must be finished to smooth,easily cleanable surfaces and seal noticeable openings on walls were noted.
Light bulbs over the cooking equipments must be shielded and other light shields in the kitchen areas must be cleaned,maintained.
Multi-kitchen equipments throughout the kitchen areas must be properly stored and inverted to prevent contamination before use.All clean single service utensils must be stored with handles up.
Cutting boards with deep, dark grooves must be sanded/bleached or replaced.
All prepared food items held inside the refrigeration units must be properly labeled,dated.Any open cans of food must be transferred into regular containers.
Hair restraints are needed for all food handlers working in the kitchen.
