All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. EMPLOYEES MUST NOT STORE PERSONAL BELONGINGS IN FOOD AND FOOD CONTACT AREAS.
All food not stored in the original container shall be stored in properly labeled containers. ALL PREPARED FOODS IN FREEZER MUST BE LABELED AND DATED. ALL BULK FOOD CONTAINERS MUST BE LABELED.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. ALL PREPARED FOODS IN FREEZERS AND COOLERS MUST BE STORED IN PROPER CONTAINERS AND COVERED.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST PROVIDE LIDS FOR BULK FOOD CONTAINERS. MUST PROVIDE PROPER LID FOR GARBAGE CAN WITH HANDLE, NO RAW, DIRTY WOOD. MUST NOT USE CARDBOARD AS LINER FOR SHELVING IN REACH IN COOLERS. ALL METAL SHELVING ON COOKS LINE, UNDER MICROWAVES, UNDER METAL PREP TABLE AND STEAM TABLE MUST CLEAN AND MAINTAIN. BROKEN RUBBER GASKET ON LEFT DOOR OF COOKS LINE COOLER, MUST REPLACE.

Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. ALL PLUMBING FIXTURES IN BOTH WASHROOMS MUST CLEAN, IE: TOILETS, WASHBOWLS ETC. MUST REMOVE THREADED NOZZLE FROM UTILITY MOP SINK OR INSTALL A BACK FLOW PREVENTION DEVICE.

MANAGEMENT DISCARDED THE CONTAINER OF BEAN SPROUTS. MANAGEMENT INSTRUCTED TO KEEP RAW FOODS AND READY TO EAT FOODS SEPARATE FROM EACH OTHER. CRITICAL VIOLATION 7-38-005A.
OBSERVED FOODS NOT PROTECTED DURING STORAGE IN THE FREEZERS AND COOLERS. OBSERVED CRAB RANGOON STORED IN CARDBOARD BOXES IN THE FREEZER AND EGG ROLLS AND SPRING ROLLS STORED IN DIRTY PLASTIC CONTAINERS THAT CANNOT BE CLEANED OR SANITIZED. ALSO OBSERVED FROZEN MEAT SKEWERS IN CONTACT WITH DIRTY CARDBOARD BOXES AND GARBAGE BAGS IN THE FREEZER. MANAGEMENT VOLUNTARILY DISCARDED ALL FOODS IN CONTACT WITH NON-FOOD GRADE STORAGE MATERIALS. SERIOUS VIOLATION 7-38-005A.
DETAIL CLEAN THE FOLLOWING: TOP OF STOVE, VENTILATION HOOD AND FILTERS, STEAM TABLE, STORAGE SHELVES AND WIRE STORAGE RACKS THROUGHOUT, PREP TABLES, TOP OF WOK STATION WITH ENCRUSTED FOOD DEBRIS.
DETAIL CLEAN THE COBWEBS ON THE CEILING AND WALLS AROUND THE PREP AREA.
REPAIR THE TORN DOOR GASKET INSIDE OF THE 2 DOOR PREP COOLER. OBSERVED FROZEN MEATS STORED IN PLASTIC GROCERY BAGS. MUST USE FOOD GRADE STORAGE BAGS ONLY TO STORE FOODS.
