THE HEAVILY RUSTED GREASE TRAP AT THE 4-COMP SINK IN REAR MUST BE REPLACED; UPPER DOOR HINGE OF FRONT COOLER BY JUICERS MUST BE FIXED- TO ALIGN THE DOOR. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair.
MISSING BASEBOARD TILES RIGHT BELOW THE WALK-IN FREEZER DOOR MUST BE REPLACED OR RE-SURFACED TO MAKE SMOOTH/EASILY CLEANABLE. The walls and ceilings shall be in good repair and easily cleaned.
OBSERVED WATER LEAKS AT ALL FAUCET FIXTURES OF THE 4-COMP SINK-MUST FIX. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.
