OBSERVED THE FOLLOWING ITEMS IN THE CHEF'S KITCHEN SANDWICH COOLER: HUMMUS AT 44.9F, CHEESE AT 46.4F, YOGURT AT 43.5-45.5F, CORNED BEEF AT 49.4F, ROAST BEEF AT 45.8F, CREAM CHEESE AT 53.2F. MANAGEMENT VOLUNTARILY DISCARDED 15# OF FOOD WORTH $47. CRITICAL VIOLATION 7-38-005A.
All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED APPROXIMATELY 100 MOUSE DROPPINGS IN THE 'PEAPOD' STORAGE ROOM AND OSCO STORAGE AREA AND HALLWAYS. MANAGEMENT INSTRUCTED TO CLEAN AND SANITIZE AFFECTED AREAS. MUST MOVE PRODUCT AND EQUIPMENT AWAY FROM WALLS AND REMOVE UNUSED EQUIPMENT TO PREVENT PEST HARBORAGE AREAS. ADDITIONAL PEST CONTROL SERVICE IS RECOMMENDED. SERIOUS VIOLATION 7-38-020.
OBSERVED STAINED CUTTING BOARD IN THE CHEF'S KITCHEN SANDWICH PREP AREA. INSTRUCTED MANAGEMENT TO REPLACE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. DETAIL CLEAN THE FLOOR OF THE DAIRY WALK-IN COOLER ESPECIALLY UNDER THE MILK RACKS.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED GAPS AND WALL OPENINGS IN THE BASEBOARD AND WALLS OF THE 'PEAPOD' STORAGE ROOM. INSTRUCTED MANAGEMENT TO SEAL ALL GAPS AND OPENINGS AND REPAIR ANY HOLES IN THE DRYWALL.
REPAIR THE WATER LEAK BENEATH THE EXPOSED HANDSINK IN THE MIDDLE OF THE DELI AREA.

Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. NO LIGHT SHIELD PROVIDED ABOVE THE 3 COMPARTMENT SINK IN PRODUCE STORAGE.PREP AREA.INSTRUCTED TO PROVIDED IT.
The walls and ceilings shall be in good repair and easily cleaned. OBSERVED PEELING PAINT AND RUSTED METAL STRIP ABOVE THE DISH MACHINE(BAKERY).INSTRUCTED TO REMOVE PEELING PAINT,PAINT AND REPLACE METAL STRIPS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. PROVIDE UNTHREAD FAUCET FOR THE 3 COMPARTMENT SINK IN MEAT REPACKING AREA.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. SWEEP FLOOR THROUGHOUT THE STORAGE AREA(UNDER SHELVES), ALSO IN WALK-IN COOLERS UNDER SHELVE(EXCESS DEBRIS AND ENCRUSTED FOOD).DETAIL CLEAN BLOOD ON FLOOR IN MEAT WALK-IN COOLER,INSTRUCTED TO PROVIDE SHEET PAN UNDER THE MEAT BOXES,FOR BLOOD TO DRIP IN IT,NOT ON FLOOR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. REPLACE BROKEN SHELVES (EAST SIDE) IN MEAT DEPARTMENT.ADDITIONAL THRESHOLD IS NEEDED AT FRONT DOOR.

DURING INSPECTION OBSERVED AN ORANGE JUICING MACHINING IN THE PRODUCE DEPT. COOLER USED IN MANUAL CAPPING PLASTIC BOTTLES OF ORANGE JUICE WHICH IS IN VIOLATION OF THE CITY OF CHICAGO HEALTH CODE.ALL BOTTLED ORANGE JUICE WAS DISCARDED WILLINGLY BY MANAGEMENT. ORANGE JUICING MACHINE WAS TAGGED AT THIS TIME. PLEASE CONTACT CDPH TO UNTAG MACHINE. SERIOUS VIOLATION ISSUED 7-38-005(A).
MUST INCREASE WATER PRESSURE AT THE HANDWASHING SINK IN THE CHEESE SHOP DEPT AND AT THE MEAT CUTTING ROOM HANDSINK AS WELL.
MUST CLEAN THE FOLLOWING ITEMS; HANDSINKS AT THE DELI AREA, PANINI MACHINE TO REMOVE ENCRUSTED FOOD DEBRIS, AND EXTERIOR OF OVEN BASE TO REMOVE GREASE BUILD UP.
MUST SEAL OR PAINT RAW WOOD WALL IN THE REAR SODA BEVERAGE AREA SO AS TO BE SMOOTH AND EASILY CLEANABLE.
