OBSERVED APPROXIMATELY 100 MOUSE DROPPINGS IN THE 'PEAPOD' STORAGE ROOM AND OSCO STORAGE AREA AND HALLWAYS. MANAGEMENT INSTRUCTED TO CLEAN AND SANITIZE AFFECTED AREAS. MUST MOVE PRODUCT AND EQUIPMENT AWAY FROM WALLS AND REMOVE UNUSED EQUIPMENT TO PREVENT PEST HARBORAGE AREAS. ADDITIONAL PEST CONTROL SERVICE IS RECOMMENDED. SERIOUS VIOLATION 7-38-020.

COOLER USED IN MANUAL CAPPING PLASTIC BOTTLES OF ORANGE JUICE WHICH IS IN VIOLATION OF THE CITY OF CHICAGO HEALTH CODE.ALL BOTTLED ORANGE JUICE WAS DISCARDED WILLINGLY BY MANAGEMENT. ORANGE JUICING MACHINE WAS TAGGED AT THIS TIME. PLEASE CONTACT CDPH TO UNTAG MACHINE. SERIOUS VIOLATION ISSUED 7-38-005(A).
