OBSERVED APPROX. 20 LIVE FRUIT FLIES BEHIND THE BAR AND IN THE BASEMENT DISHWASH AREA. ADDITIONAL PEST CONTROL SERVICE IS NEEDED TO ELIMINATE ALL PEST ACTIVITY. SERIOUS VIOLATION 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. DETAIL CLEAN THE FOLLOWING: GREASE BUILDUP ON VENTILATION HOOD, FOOD DEBRIS AT BOTTOM SHELF OF PREP TABLE, INTERIOR OF MICROWAVE.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. MAINTAIN THE FLOOR DRY BEHIND THE BAR TO PREVENT FRUIT FLY ACTIVITY. DETAIL CLEAN THE FLOOR OF THE 1ST FLOOR PREP AREA.

LEAK UNDER FOOD PREP EXPOSED HAND SINK, LEAK AT NOZZLE ON BASEMENT THREE COMPARTMENT SINK BOTH REPAIRED WITH DUCT TAPE. MUST PROPERLY REPAIR AND REMOVE ALL DUCT TAPE. CONSTANT RUNNING LEAK UNDER BASEMENT MOP SINK ONTO FLOOR AND DOWN A STEP INTO A NEAR BY FLOOR DRAIN. MUST REPAIR.
ICE SCOOPS AND ALL COOKS LINE WIPING CLOTHS MUST BE STORED IN A CLEAN CONTAINER WITH A SANITIZING SOLUTION.
