INSTRUCTED TO REPAIR LEAKY DRAINPIPE UNDER 3RD COMPARTMENT OF THE 3 COMPARTMENT SINK IN REAR AREA.---- MUST PROVIDE 3 WORKING DRAIN STOPPERS AT 3 COMPARTMENT SINK IN REAR.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. NOTED READY TO EAT FOOD STORED IN PLASTIC BAG (REACH-IN FREEZER)AND IN STEAM TABLE. INSTRUCTED TO PROPERLY STORED IN FOOD GRADE CONTAINER.----NOTED BULK RICE STORED IN GARBAGE CONTAINER. INSTRUCTED TO PROPERLY STORED IN A FOOD GRADE CONTAINER.

FOUND INADEQUATE PEST CONTROL I.E INCOMPLETE PEST CONTROL LOG BOOK DURING THIS INSPECTION MISSING DOCUMENTS LICENSURES,A MAP OF ALL LOCATIONS OF ALL APPLICATIONS OF PESTICIDES,TRAP,AND MONITORING DEVICES.ALSO FOUND AN EXHAUST FAN IN KITCHEN NOT INSECT PROOFED.MUST PROVIDE A COMPLETE LOG BOOK AS REQUIRED,INSECT PROOF SAID EXHAUST FAN IN KITCHEN.SERIOUS VIOLATION 7-38-020.
All dishwashing machines must be of a type that complies with all requirements of the plumbing section of the Municipal Code of Chicago and Rules and Regulation of the Board of Health. FOUND THE 3 COMP SINK NOT MAINTAINED THAT IS DRAINING SLOWLY ALLOWING WASTE WATER TO COME BACK INTO OTHER COMPARTMENTS WHEN MIDDLE COMPARTMENT IS DRAINING.MUST REPAIR AS REQUIRED. SERIOUS VIOLATION 7-38-030.
All food not stored in the original container shall be stored in properly labeled containers.BULK FOOD CONTAINERS IN THE DRY STORAGE ROOM MUST BE LABELED TO IDENTIFY FOODS STORED.
The walls and ceilings shall be in good repair and easily cleaned.SEAL NOTICEABLE OPENINGS AT KITCHEN AREA ON WALL BEHIND THE FOOD PREP TABLE.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline.
