Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health.FOUND NO BACKFLOW PREVENTION DEVICE AT ICE MACHINE LOCATED IN BASEMENT DRY STORAGE AREA.
The walls and ceilings shall be in good repair and easily cleaned.FOUND WALLS IN BASEMENT DRY STORAGE AREA WITH PEELING DEBRI MUST REPAIR TO SMOOTH,EASILY CLEANABLE SURFACES.

All food not stored in the original container shall be stored in properly labeled containers. All prepared food stored in coolers must be dated & labeled.

Must repair water leak at pipe beneath exposed handsink,a backfloww prevention device is needed at mop sink.
All employees shall be required to use effective hair restraints to confine hair. Food handlers must have hair restraints when working around open food.

Must provide,maintain 3 working sink stoppers for the 3comp sink,replace/repair rusty shelvings inside 2 door stand-up cooler using a non-toxic finish.
Must provide,maintain working thermometers inside the refrigeration units at all times.
