Previous minor violations not corrected from 7-24-2009, report # 156571: -33 Must clean all shelves, coolers shelves, toilet bowl and utility sink. -34 Must clean floors behind shelves and cooler and washroom. -35 Must clean walls, ceiling of washroom. -37 Must change worn out spring ddor of toilet. -40 Must provide thermometer in all coolers. Serious citation issued, 7-42-090.
Evidence of rodent infestation found through-out premises. Found hundreds of mice droppings under display shelves, in rear storage room, under 2 compartment sink, at front register areas, in front closet, on electrical boxes and along wallbases. Also, outer openings not rodent or insect proofed. Front and rear exit doors not rodent proofed at bottom. Instructed to rodent proofed doors to prevent rodent and insect entry. Must have licensed pest control service premises and provide current service invoice with all required documents. Critical citation issued 7-38-020. Last pest control service report dated 7/23/10. Must update information in log book.
Hot running water not provided under city pressure. Premises has 6 gallon hot water tank, outlet pipe cool to the touch. Instructed that establishment must provide hot running water under city pressure at all times at all sinks. Serious citation issued, 7-38-030.
No running water on leftside of hand sink in toilet room. Instructed to provide hot and cold running water. Handsink in toilet room leaking at bottom drainline. Instructed to repair to prevent leak.
Unnecessary clutter (crates, buckets, weeds etc) outside rear of premises. Instructed to remove all unused articles and debris. Keep area clean. Also, remove all unnecessary and unused items (hubcaps,old/dirty shelves, machines etc)from inside premises. Store all else to prevent pest harborage and to facilitate cleaning.
Ventilation covers wall and ceiling not cleaned. Instructed to detail clean. Clean and repair walls through-out, seal all holes and openings.
No raised shelving in walk-in cooler, beer/liquor stored on floor. Must provide raised shelving to keep product 6 inches off floor.
