FOLLOWING NEEDS CLEANING AND SANITIZING: INTERIOR/EXTERIOR OF ALL EQUIPMENT, MIXER, LEGS OF TABLES,WOODS,GASKETS INSIDE REFRIGERATION AND WALK-IN COOLER/FREEZER UNTIS, FAN GUARDS INSIDE REFRIGERATION UNITS AND WALK-IN COOLER, INTERIOR PANEL OF ICE MACHINE, WALLS AT BAR AND PREP AREAS TO REMOVE SODA SYRUP SPILLS/FOOD SPLATTERS, SODA GUN HOLDER, DRY FOOD STORAGE ROOMS, AROUND PALLETS, SHELVES/RACKS, VENTILATION HOOD/FILTERS, VENT COVERS, INTERIOR/EXTERIOR OF ONE COMPARTMENT SINK BY DISH MACHINE, THREE COMPARTMENT SINK/DRAINS, AROUND GREASE TRAPS, UNDER, AROUND ALL EQUIPMENT THROUGHOUT PREMISES TO REMOVE DARK STAINS BUILDUP.

MANAGER CALLED ECOLAB TO SERVICE UNIT. INSTRUCTED TO WASH, RINSE, AND SANITIZE MULTI-USE UTENSILS AND DISHES AT THREE COMPARTMENT SINK. ECOLAB ARRIVED ON SITE TO SERVICE UNIT AROUND 1:30PM-STATES CHECK VALVE WAS CLOGGED-REPLACED CHECK VALVE AND CHLORINE READING AT 100PPM AT THIS TIME. CRITICAL 7-38-030 ISSUED.
FOUND SCREEN DOORS AND SCREEN AT WINDOW AT KITCHEN AND DRY FOOD STORAGE ROOM NOT TIGHT FITTING. 1/2 INCH GAPS NOTED AT TOP AND SIDES OF SAID DOORS AND WINDOW. INSTRUCTED TO MAKE TIGHT FITTING SO AS NO DAYLIGHT CAN BE SEEN. ALSO, NOTED 12 MOUSE DROPPINGS IN DRY FOOD STORAGE ROOM AT BOTTOM SHELF AND 10 MOUSE DROPPINGS INSIDE CABINET UNDER WASHBOWL IN STAFF TOILET ROOM. INSTRUCTED TO CLEAN, SANITIZE AFFECTED AREAS, AND RECOMMEND PEST CONTROL TO SERVICE PREMISES. SERIOUS 7-38-020 ISSUED.

PROVIDE SPLASH GUARD BETWEEN HAND WASH SINK AND DISH MACHINE AREA-REMOVE CARDBOARD. REMOVE ALL LINING, ALUMINUM, CLOTH, AND CARDBORAD FROM ALL PREP TABLE BOTTOMS, EQUIPMENT, AND SHELVES AT BAR STORING GLASSWARE. PAINT RUSTY PREP TABLE BOTTOMS. REPLACE SOILED GARBAGE BINS IN KITCHEN. DEFROST CHEST FREEZERS.

INSTRUCTED TO PROVIDE HOT AND OR TEMPERED RUNNING WATER PER CITY CODE AT ALL TIMES, REMOVE BEVERAGE STAINS FROM INTERIOR HAND WASH SINK, AND REMOVE PREP TABLE FROM BLOCKING HAND WASH SINK TO MAKE ACCESSIBLE FOR HAND WASHING BY EMPLOYEES. EMPLOYEE TURNED ON HOT WATER VALVE TO PROVIDE HOT RUNNING WATER, PREP TABLE REMOVED, AND SINK CLEANED TO REMOVE STAINS. CRITICAL 7-38-030 ISSUED.
FOUND SODA LINES STORED IN DIRECT CONTACT OF ICE INSIDE UNCOVERED ICE BIN AT BAR AREA. INSTRUCTED TO PROVIDE PARTITION BETWEEN SODA LINES AND ICE BEING USED TO SERVE CUSTOMERS AS WELL AS COVER/LID TO PROTECT ICE FROM CONTAMINTATION. ALSO, NOTED DARK, SLIMY BUILDUP COVERING WATER DISPENSER NOZZLES LEAKING INTO WATER PITCHERS NEXT TO BAR AREA BEING USED TO SERVE CUSTOMERS ICE WATER. INSTRUCTED TO DISCARD ICE FROM ICE BIN, CLEAN, AND SANITIZE WATER DISPENSER NOZZLES, REMOVE ICE WATER, PROVIDE PARTITION AND COVER/LID FOR ICE BIN AT BAR. SERIOUS 7-38-005 (A) ISSUED.
