Unless something has changed in the last 10-15 years, I don't know why he'd need to take differential calculus again to have a better understanding of what's in store for him with integral calculus.

However, he should do whatever he's more comfortable doing.

Depending on the school, testing out of Calc 1 could be a big deal. There are so many recommended and required courses to take for health science professional schools that the additional free three credit hours could allow your son to take something that fits in the "useful but not required" category.

I retook calc 1 because I was worried that "engineering calc 1" would be much harder than high school. It was difficult but easier than many other math classes I took. It was a great "warm-up" for the college style of learning/testing. I would much rather have an easier class to help me adjust to the college way of doing things instead of adding one more thing to overwhelm me.

Just my .02

Are you stressing because he will be moving away for school? Don't worry - he will keep in touch. I can tell.
Well....since he asked me to help I feel compelled to do so.

My vote is for calc1, one class isn't the end of the world and it would be less stressful too.
His mom is not picking classes for him. He asked for my help and my opinion.

The kids who played on team sports... who are often channeled into taking easier courses as they are training/playing so much.

But you said they got 4 or 5 on the AP Calculus exam. So they must have been taking at least one hard class.
