Fortunately, I believe the calls for reform of Obamacare will come most loudly from Democrats as the 2014 elections draw nearer--resulting in a bipartisan approach to fix this mess. Otherwise the Dems will get slaughtered at the polls.
That's ridiculous. Democrats are ready to make ACA betternow. It's the Republicans who will refuse to do anything other than repeal, defund or otherwise do away with provisions of ACA. The stalemate will remain.

Better how? Magically come up with doctors willing to accept $24 in reimbursement rates? Magically come up with enough doctors to begin with? 70% of California doctors aren't even going to participate.
This does not play out money-wise. What exactly do they think will fix it?

My parents' doctor is charging a $3000 a year per patient fee on top of accepting medicare and regular insurance for the pleasure going to him. My parents can afford to pay it and are thrilled that its easier now to get an appt. I think the side effect of some of this will be that doctors start to charge fees like this in order to supplement their lower reimbursement rates.

Why? It's his business he can charge what he likes. Doctors aren't required by law to accept any insurance or agree to any particular rates, they are not (yet) government employees. You can always choose to take your business elsewhere if you don't like his terms.

My point is that often these programs have the opposite effect of what they intend. The ACA may not wind up equalizing healthcare but instead create healthcare for the wealthy and then healthcare for everybody else. Where I grew up in Canada (in a wealthy suburb of Toronto) we had single payer insurance but my neighbors still flew to the Mayo Clinic and paid cash if they got seriously ill.

Many of California's doctors will not be participating in either the MediCal expansion or the CoveredCalifornia plans, both of which have very low reimbursement rates. There has been talk about this here for some time--there are parts of the state that already don't have enough doctors for the people that already had insurance, and adding thousands of people onto MediCal will not magically make new doctors appear.
Probably why our governor recently signed a bill allowing non-doctors to dramatically increase the types of procedures that they could do.
It is called "Concierge Medicine." Pay extra, and get more individualized attention. Appointments same day, etc. It is starting to become common.
