- Drying mops
Observation: Mop was found wet in empty mop bucket. If not in immediate use the mop must be allowed to air dry to prevent buildup of bacteria. All should be air dried unless they are stored in a sanitizing solution. Corrected on site.
- Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
Observation: Spills or drippage on floors under milk and eggs reach-in cooler shelves or in walk-in cooler. All area must be clean to ensure that cleaning is effectively implemented throughout all areas of food business, cleaning needs to planned and organized and stuff must understand and know responsibilities. This is repeat violation.
- Outer openings are protected
Observation: Back receiving door has light showing on the bottom arround on one side. Manager said they fixed it, but only one side. Premise must be protected against the entry of insects and rodents. Maintain door seal. Check for correct door fit with weather stripping for a tight seal on bottom of the door and nstall a door sweep on another side to seal the bottom.
- Light intensity
Observation: Missing light bulb or burnt out in ice cream reach-in store cooler. Must provide adequate light intensity in the area where is food stored inside reach-in refrigerators for easily cleaning and maintaining.

- Food storage - preventing contamination from the premises
Observation:Single servings of candy bars have fallen out of display on cooler door and are now on floor in soda area, all foods need to be stored at least 6 inches off floor.
- Physical facilities maintained in good repair
Observation:Complaint received about hole in carpet in pharmacy area, carpet at pick up counter is worn but no visible hole exposing subflooring etc. Management has reported need for new carpet in this area.

Observation:Dock receiving door has gaps where light showing through when fully closed, seal to prevent pests from entering, wherever light showing through.

Observation:1)Floor in walk in cooler under milk racks is soiled, clean. Paper debris also on floor in cooler, clean. 2)Tile floor along coolers is soiled, clean. 3)Various areas of carpeted areas have evidence of paper debris. Manager reports new vacuums have arrived today and will be used tonight, other vacuums have not been performing well recently.
