      Some of the condiments (sesame and sunflower seeds) and the chocolate brownies at the salad bar are not under the sneeze guard. Rearrange items or move some to another area so everything is adequately protected from contamination under the sneeze guard.
    • Labeling (Food not properly labeled 3-602.11, p 93 IAC 481-31.5)
      Provide ingredient labels for all foods not prepared on site. This includes the cookies, rice krispie bars, scones, muffins, brownies. Can label each individual product or post a sign that says "ingredients are available upon request" and provide a notebook with the ingredients or post the ingredients on the display line.

    • Adequate warewashing facilities (No test kit for checking concentration of sanitizer 4-302.14, p 120)
      Provide quat test strips to check the sanitizer concentration in the rag buckets and 3 compartment sink. Strips have been ordered.

      Deli meats in self serve cooler were at 50-55 degrees. All cold food shall be held at 41 degrees or less. Discarded per request. Remainder of items in cooler were at 41 degrees or less. Manager thinks meat was held at room temperature too long before being placed in cooler. Will speak with employees about the procedure used. Replacement meats temped at less than 41 degrees. Corrected during inspection.

      Shields had been removed from one side of the self serve food table and had not been replaced, allowing for potential of food contamnation. Corrected during inspection.

      Need to provide ingredient list for self serve cookies at the food bar area.

      Can opener has buildup and needs to be cleaned more frequently.

      Some items in dry storage were being stored on cardboard which is not a cleanable surface. Removed and corrected during inspection.

      Employee coat stored next to food items. Moved and corrected during inspection.

      The bins of flour and sugar in the kitchen were not labeled. The bottle of oil was not labeled. Label items with common name. Corrected on site.

      The sanitizer bucket in the dry storage room did not have adequate concentration of sanitizer. Replentish the sanitizer supply. Corrected on site.

    • Food contact surfaces (Storage: equipment/utensils, single service & use items 4-903.11, p 141)
      Single use plates and silverware at the serving line were not stored inverted. Corrected on site. Unwrapped single use portion cups stored with baking utensils/supplies. Wrap the portion cups or relocate them to another area. Single service items shall be stored inverted or wrapped and in a location that they are not exposed to contamination. Corrected on site.

      Clean the dried spill under the coffee area. Clean the storage area under the soda that used to hold the sweeters. Some residue remaining from the sweetener.
