February 21, 2007 (Complaint)

Comments:
Visited facility in response to customer complaint that on 2/13/07 about 4:00-4:30 p.m., customer was served fish and shrimp dinner, customer bit into a white pill with very bitter taste. Shrimp is shelled and deveined, Japanese bread crumbs and oyster breader is mixed and shrimp is pressed into the coating. These shrimp are stored in walk-in box then the tray is carried to the refrigerator at cook line as needed. No medications were observed in prep or cook area. Tylenol caplets and aspirin were in bottles on managers desk. Manager did not have any knowledge of any employees personal medications being in prep/cook area. Observation of shrimp making process and visual evaluation of shrimp in walk-in box did not reveal any abnormalities.

Visited facility in response to customer complaint that on 1/16/07 around 1:00 p.m. customer found 2 strands of hair cooked into country style fish. Observed some employees with unrestrained hair.     Discard.

  • 0820 - Corrected During Inspection Critical Repeat Temp of PHF in walk in above 45 degrees (fish 54 degrees, crab 59 degrees-fish prpe 3 hours earlier). Ambient air temp was 50 degrees.
    Move PHF to colder units. Keep door closed.
  • 1800 - Shelving next to fryer not clean.
    Clean.
  • 1960 - Repeat Containers stacked while wet.
    Air dry.
  • 2720 - Repeat Doors open on dumpster.
    Close.
  • 3030 - Corrected During Inspection No towels at handsink.
    Provide.
  • 3170 - Repeat Rust on doors of walk in box and freezer.
    Resurface or repair to make smooth and easily cleanable.
  • 3180 - Repeat Ceiling over prep table splattered with pieces of shrimp.
    Clean and install guard on shrimp deveiner.
  • 3240 - Corrected During Inspection Faucets on handsink with caked on batter or breading.
    Thoroughly clean.
  • 3340 - Corrected During Inspection Critical Detergent stored next to batter mix.
    Properly store chemicals.

    Properly store.

    Keep cloth clean and in sanitizer solution.
  • 0820 - Critical Fish in walk in box at 56 degrees. Ambient air temp in unit reading 55 degrees.
    Move PHF to other refrig or freezer-keep door closed on refrig. If temp does not drop to 45 degrees or below within next hour call for repair and do not store any PHF in unit. (Temp dropped to 50 degrees withing 1/2 hour after keeping door closed).
  • 1060 - Rusty shelving.
    Replate or replace.
  • 1320 - No test kit present to check sanitizer.
    Provide.
  • 1570 - Gasket torn on sandwich unit refrig and loose gasket on walk in door.
    Repair.
  • 1780 - Critical Containers on clean end of sink with visible food debris.
    Rewash.
  • 1960 - Containers stacked while wet.
    Air dry before stacking.
  • 2000 - Single serve cups and lids stored too close to handsink and subject to splash contamination.
    Store where not subject to contamination.
  • 2720 - Doors open on dumpster.
    Keep doors closed.

    Provide.
