Leaves and other debris were found piled in the corners of the small outdoor play area, allowing for possible pest and rodent harborage. Rodent droppings were found along the wall behind the chest freezer, as well as inside of the hand sink vanity inside of the Pre K room. In the infant room no thermometer was found present at floor level. Carpet was found under the hand sink in the Pre K room, thus making the flooring absorbent, and not smooth and easily cleanable. The Pre K room was found in a soiled condition. In the Pre K Jr. room the carpet was found torn where it meets the tiling. In the Toddler 2 the tile flooring was found chipped and cracked next to the carpeting near the back door. In the Toddler 1 room the area rug was found torn. In the Girls Bathroom the coving was found unattached on the hand sink vanity. In the Boys Bathroom in-between the urinals exposed raw wood was found, making the wall area not easily cleanable. Lights throughout the facility were found not working properly or not operable. The plumbing under the hand sink in the Pre K room was found leaking into a wash tub. In the Toddler 2 room the caulking on the hand sink was found cracking and chipped. In the Toddler 2 room the toilet caulking was found cracked and chipped. In the girls bathroom toilet paper was found missing in 2 of the stalls. In the boys bathroom the caulking on the hand sink was found chipped and torn. In the boys bathroom the right urinal was found not able to flush. In the boys bathroom the left urinal was found with constant water running, for long period of time when flushed. The laundry equipment was located inside of the food preparation area of the kitchen. In the Pre K room non smooth and easily cleanable rocks and pine cones were found. In the Pre K room several of the childrens bedding was found on the floor. In Pre K Jr. room non smooth and easily cleanable tree branches and leaves were found in a tub. In the Toddler 1 room the crawling blocks were found with rips and gashes in the cover material, and were not easily cleanable. In the Infant room the disinfectant was at 200ppm of chlorine, the sanitizer was at 0ppm of chlorine. In the Pre K room unlabeled spray bottles of blue and pink chemicals were found. In the Pre K room the sanitizer was at 0ppm of chlorine. In the Toddler 2 room the disinfectant was at 200ppm of chlorine. In the Toddler 1 room the disinfectant spray bottle was at 100ppm of chlorine and the sanitizer was at 0ppm of chlorine. In the Girls Bathroom there was no sanitizer in the spray bottle. In the Boys Bathroom there was no sanitizer solution in the spray bottle. The spray bottle with blue chemical was not labeled. In the Kindergarten room the blue chemical spray bottle was not labeled. The sanitizer spray bottle in the Kindergarten was at 0ppm of chlorine. Disinfectant must be greater than 200ppm of chlorine and sanitizer must be 50 200ppm of chlorine. The staff throughout the facility did not demonstrate adequate knowledge of the proper procedures for the child care operations. The staff was observed not using proper hand washing procedures while in process of serving foods. The kitchen staff was observed using a twist top bottle for an employee beverage container. The hand sink in the kitchen did not have paper towels stocked at it. The hand sink was not accessible due to beverage cups filled the sink basin. Rodent droppings were found in the dry storage room behind the chest freezer. Childrens medication was found being stored next to beverages inside of the reach in food cooler. The one of the light panels was found not working inside of the kitchen. The plastic cups stacked in the cabinet were found stacked wet and nested. The milk food cooler was found without a thermometer. Next to the door of the kitchen a hole in the dry wall was found. The cooking utensils were found stored in the container with the food contact surface up, allowing for bare hand contact with the surface. The facility was found to have digital thermometers for checking temperatures of foods; however they had not been removed from the packaging. The staff preparing the foods was observed wearing bracelets while preparing foods for the children. Personal chili foods were found inside of the large reach in food cooler, that were not labeled or separated from childrens foods. In the infant room the staff was observed not disinfecting the diaper changing surface after changing a diaper. In the infant room the staff was observed not wearing a smock. The diaper changing pad in the Toddler 1 room was found to have rips, and torn areas on the covering, thus not allowing the surface to be smooth and easily cleanable. In the Pre K room the rabbit cage was found with urine and feces accumulation, thus creating a nuisance odor in the room. In the Girls bathroom the cabinet under the sink storing hazardous chemicals was found not locked.
