An employee beverage was stored on the food preparation table.
Shredded cheese (55�°F), cooked beef (50�°F), shredded lettuce (47�°F), cheese rellenos (49�°F), slice cheese (56�°F), ham (52�°F), and raw beef (54�°F) were all 47-55�°F in the cold-top cooler.
A thermometer was not provided to the cold-top cooler.
The floor coving was not sealed to the restroom walls. The floor tile was cracked in dry storage. Food was stored on the floor in the walk-in cooler. The walls in the walk-in cooler were soiled with mold. A cover was not provided to the floor drain below the three-compartment sink.
