The staff was not observed washing their hands before handling foods and making pizzas, or in between making the pizza and prepping food equipment.
The staff was observed using bare hands to remove foods from the dial stem thermometer before inserting the thermometer into other foods.
A basket of employee bread sticks were found stored on the back table next to food equipment in the back storage area.
The staff did not have the knowledge of how to properly calibrate the dial stem thermometers, using the ice point method.
The bottom of the cold table was found out of temperature, the alfredo sauce was at 48�°F, the meat sauce was at 47�°F, the refried beans were at 51�°F. The items were stored inside of the cooler for longer than four hours.
The hand sink in the back food preparation area near the back door did not have hot running water at the sink. The food preparation sink in the back preparation area near the back door did not have hot running water at the sink.
A large trash can was found stored directly in front of the hand sink, not allowing the sink to be easily accessible to the staff.

The walls inside of the pizza walk in cooler were found with holes, thus not allowing the walls to be smooth and easily cleanable.
The stacked metal food storage pans were found soiled with food debris. The table top mounted can opener was found soiled with food debris.
The walls and floors of the pizza walk in cooler were found soiled with food, and paper debris. The walls inside of the food walk in cooler were found soiled with food debris. The shelving in the back food preparation room was found soiled with food debris. The rolling shelving in the back food preparation area was found soiled with food debris.
The dish washing machine was found soiled with debris. The ware washing area in the facility was found soiled.
The mop sink was found with standing water in the basin, as it was not draining. The ice bin in the soft drink fountain area was found leaking directly onto the floor in the beverage serving area.
The walls, and floors throughout the food preparation, cooking, storage and ware washing area were found soiled with food and other debris. The walls in the ware washing, food preparation, cooking and storage were found with holes, thus not allowing the walls to be smooth and easily cleanable. The FRB (Fiber Reinforced Board) was found coming unattached from the walls, and not sealed to the mop sink basin.
The light inside of the walk in freezer was found not providing sufficient lighting inside of the freezer. The walk in freezer light was found without a protective cover installed.
