Carpet was present directly below the handsink and water fountain in Modular 1.
Insufficient water pressure was supplied to the water fountain in modular 1 not allowing the water stream to be above the lip guard.
Play equipment was stored in the Modular 2 restroom.
Both the disinfectant and sanitizer solutions provided to the modular had concentrations less than 50 ppm chlorine residual.
A staff member in room 128 stated that dry items in sensory tables are not changed out weekly.
