Violation descriptions and comments

The sanitizer in the three compartment sink contained a concentration greater than 400ppm quaternary ammonia residual.
A bulk spice bin under the dry storage shelf area was unlabeled as to its contents.
Cardboard being used to line the shelves in the walk in freezer was not smooth and easily cleanable. The handle on a spatula stored on the shelf above the three compartment sink was melted and no longer smooth and easily cleanable. The caulking on the food preparation sink was torn.
The caulking on the rear three compartment sink was moldy. The small front three compartment sink was not properly sealed to the wall.
The interior of the reach in cooler and the reach in freezer to the right of the fryer was soiled.
The fryer units were soiled with grime. The faucet on the large rear three compartment sink was soiled with grime. The shelves above the three compartment sink were soiled with grease residue.
The caulking on the front handsink was torn. The mopsink was soiled with debris. The faucet in the Mens restroom was loose.
The wall behind the french fry hot hold unit was severely soiled with grime. The wall behind the hamburger patty reach in freezer was soiled with debris. The wall below the rear handsink was in disrepair. The wall above the mopsink was soiled with grime. The floor drain below the large rear three compartment sink was soiled with food residue. The floor under the storage rack to the right of the automatic ice machine was soiled with debris. The ceiling vent above the shake machine was soiled with dust.
The walk in cooler was not provided with the required 10 foot candles of light sufficient for proper food identification. The light in the Mens restroom was not functioning adequately.
