The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. INSTRUCTED TO CLEAN FLOORS ALONG THE WALLS AND IN ALL CORNERS THROUGHOUT PREMISE ESPECIALLY UNDER AND AROUND COOKING EQUIPMENT, COOLERS AND FREEZERS.
The walls and ceilings shall be in good repair and easily cleaned. INSTRUCTED TO CLEAN WALLS BEHIND EXPOSED HANDSINK WITH DRIED FOOD SPILLAGE.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. INSTRUCTED TO REPAIR BROKEN VENTILATION INSIDE WASHROOM IN REAR AREA.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. INSTRUCTED TO PROVIDE THERMOMETER VISIBLE INSIDE COOLERS AND FREEZER.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. INSTRUCTED TO REMOVE ALL UNNECESSARY ARTICLES, BOXES STORED IN REAR STORAGE ROOM TO PREVENT RODENT AND INSECT HARBORAGE.----------ALL USABLE ITEMS MUST BE STORED 6' OFF THE FLOOR FOR EASY ACCESS ON CLEANING.-------MUST REORGANIZE.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. NOTED ICE SCOOP STORED ON TOP OF ICE MACHINE. INSTRUCTED TO PROPERLY STORED IN A CLEAN SEPARATE CONTAINER.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. INSTRUCTED TO CLEAN AND MAINTAIN THE FOLLOWING: INTERIOR AND EXTERIOR SURFACES OF EXPOSED HAND SINK, 3 COMPARTMENT SINK, EXTERIOR AND INTERIOR SURFACE OF ICE MACHINE BEHIND UPPER PANEL, INTERIOR SURFACES OF PREP COOLER AND REACH-IN COOLER IN FOOD PREP AREA.

Facilities not maintaining proper temperature. Found internal temprature of prep cooler at improper temperature-air temperature at 51.0 F while storing potentially hazardous foods. Instructed that coolers must maintain 40 F or below. Critical citation issued 7-38-005(A). 'Held for Inspection' tag placed on cooler.
Potentially hazardous foods does not meet temperature requirement during storage, preparation, display and service. Found 1 lb of Chorizo at 49.3 F, 1/2 lb of Corn Beef Hash at 61.0 F, 1 lb of cooked Ground Beef at 61.2 F and various condiments. Instructed that potentially hazardous foods must maintain 40 F or below during storage. Manager voluntarily denatured and discarded. Value $20, 2 1/2 lbs. Critical citation issued 7-38-005(A).
