All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. THE FOLLOWING DOORS MUST BE RODENT PROOF WITH TIGHT FITTING SEAL ON BOTTOM: FRONT ENTRANCE/EXIT DOOR. REAR EXIT DOOR TO OUTSIDE GARBAGE. FAR REAR PREP AREA DOOR. DOOR TO ADJACENT BOOK STORE MUST BE TIGHT FITTING ON SIDE. All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST INSTALL SPLASH GUARD BETWEEN EXPOSED HAND SINK AND THREE COMPARTMENT SINK AT FRONT BAR/PREP AND FAR REAR PREP AREA. MUST PROVIDE LIDS FOR BAR ICE BINS. The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GREASE RECEPTACLE DIRTY WITH GREASE SPILLAGE. MUST CLEAN. MUST CLEAN. WOMANS WASHROOM RIGHT SIDE WASHBOWL AUTOMATIC NOZZLE DOES NOT STAY ON. MUST REPAIR. The walls and ceilings shall be in good repair and easily cleaned. ALL HOLES IN WALLS AND OPENINGS AROUND PIPING IN ALL AREAS MUST BE SEALED. Shielding to protect against broken glass falling into food shall be provided for all artificial lighting sources in preparation, service, and display facilities. MUST PROVIDE LIGHT SHIELDS THROUGHOUT BASEMENT PREP. Smoking shall be prohibited in all public places except in designated smoking areas. 'NO SMOKING' SIGNAGE MUST BE POSTED AT ENTRANCE TO ESTABLISHMENT.

All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. OBSERVED DARK PITS ON SURFACE OF CUTTING BOARD AT 1ST FLOOR PREP AREA BY OVEN. MUST REPLACE DAMAGED CUTTING BOARD. All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. OBSERVED FOOD BUILD UP INSIDE COOLERS. MUST CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN COOLERS.
