MUST REMOVE ALL PLASTIC CRATES AND PROVIDE ADDITIONAL SHELVES TO ELEVATE ITEMS OFF FLOOR TO PREVENT POTENTIAL PEST HARBORAGE. INSTRUCTED MANAGER TO DETAIL CLEAN ALL UNCLEAN EQUIPMENT AND MAINTAIN ALSO MUST CLEAN LOWER SIDES OF ALL PREP TABLES/SHELEVES. The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED FOOD DEBRIS ON FLOOR UNDER AND AROUND ALL EQUIPMENT, ALONG ALL WALLS ALSO OBSERVED STANDING WATER ON FLOOR UNDER COOKING EQUIPMENT AND BY GREASE TRAP. MUST DETAIL CLEAN FLOOR/KEEP FLOOR DRY THROUGHOUT. The walls and ceilings shall be in good repair and easily cleaned. OBSERVED PEELING PAINT ON CEILING TILES ABOVE PREP AREA AND ABOVE DISHMACHINE. MUST REPLACE ALL DAMAGED CEILING TILES. Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 2-COMPARTMENT SINK FAUCET LEAKING AT BASE.
